【摘要】：目的探討Silver Hawk腔內(nèi)斑塊切除術(shù)治療膝下動脈硬化閉塞性疾病的安全性和有效性。方法回顧性分析28例采用Silver Hawk斑塊切除術(shù)治療膝下動脈硬化閉塞癥患者臨床資料,通過血管多普勒超聲和CT血管成像評價管腔通暢情況,記錄患者手術(shù)前后踝肱指數(shù)(ABI)、并發(fā)癥發(fā)生情況以及靶血管一、二期通暢率。結(jié)果28例患者手術(shù)均獲得成功。全組無圍手術(shù)期死亡病例,術(shù)后所有患者臨床癥狀較術(shù)前明顯改善,術(shù)后第2天ABI1.09±0.25與術(shù)前ABI 0.52±0.17差異有統(tǒng)計學(xué)意義(P0.05)。術(shù)后6、12、24個月一期通暢率分別為76.47%、70.59%、64.71%,二期通暢率分別為94.12%、91.18%、82.35%。潰瘍愈合率87.5%,其中5例缺血性潰瘍患者潰瘍逐漸愈合;2例壞疽患者足趾脫落,創(chuàng)面愈合,1例壞疽患者因感染行膝下截肢,截肢平面較術(shù)前下降。結(jié)論 Silver Hawk腔內(nèi)斑塊切除術(shù)治療膝下動脈硬化閉塞性疾病是一種安全有效的方式,近、中期效果滿意。
[Abstract]:Objective to evaluate the safety and efficacy of Silver Hawk endovascular plaque resection in the treatment of inferior knee arteriosclerosis obliterans.Methods the clinical data of 28 patients with subgenu arteriosclerosis obliteration treated by Silver Hawk plaque resection were retrospectively analyzed. The patency of the lumen was evaluated by Doppler ultrasound and CT angiography.The malleolus brachial index (ABI), the incidence of complications and the patency rate of target vessels were recorded before and after operation.Results all the 28 patients were successfully operated.There were no perioperative death cases in the whole group. The clinical symptoms of all the patients were significantly improved compared with those before operation. The difference of ABI1.09 鹵0. 25 and ABI 0. 52 鹵0. 17 on the second day after operation was statistically significant (P 0. 05).The primary patency rates were 76.47 and 70.591.The second stage patency rates were 94.121,91.18 and 82.35, respectively.The healing rate of ulcers was 87.5%. Among them, 5 cases of ischemic ulcer patients had ulcer healing and 2 cases of gangrene patients had lost their toes, and 1 case of gangrene patients had undergone subknee amputation due to infection. The level of amputation was lower than that before operation.Conclusion Silver Hawk endovascular plaque resection is a safe and effective method for the treatment of inferior knee arteriosclerotic occlusive disease.
