【摘要】：目的:探討纖維蛋白膠(fibrin glue,FG)在全膝關(guān)節(jié)置換術(shù)(total knee arthroplasty,TKA)中應(yīng)用的有效性和安全性。方法:通過檢索Pub Med、Embase、Cochrane Library、CNKI、CBM、萬方等數(shù)據(jù)庫(kù),同時(shí)手工檢索圖書館館藏期刊,由2位研究者按照納入和排除標(biāo)準(zhǔn)獨(dú)立篩選高質(zhì)量的隨機(jī)對(duì)照試驗(yàn)(randomized controlled trials,RCTs)文獻(xiàn),采用Rev Man 5.2軟件對(duì)提取的數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:共納入11個(gè)RCTs,共973個(gè)TKA患者。Meta分析結(jié)果顯示:與對(duì)照組相比,纖維蛋白膠可以減少TKA術(shù)后引流(MD=-227.36,95%CI=-359.47~-95.25,P=0.001);血紅蛋白丟失量(MD=-0.63,95%CI=-0.79~-0.46,P=0.000);總失血量(MD=-235.59,95%CI=-453.31~-17.86,P=0.030);輸血率(OR=0.44,95%CI=0.31~0.61,P=0.000);且不增加術(shù)后并發(fā)癥的發(fā)生(OR=0.94,95%CI=0.61~1.45,P=0.790)。結(jié)論:FG可以有效減少TKA術(shù)后引流量,總失血量,血紅蛋白丟失量,輸血率但不增加術(shù)后并發(fā)癥的發(fā)生。
