【摘要】：目的人工全膝關(guān)節(jié)置換術(shù)是目前治療各類終末期或重癥膝關(guān)節(jié)病變最有效、最成功的手術(shù)之一,但目前對于全膝關(guān)節(jié)置換術(shù)中是否常規(guī)行髕骨置換尚未達(dá)成一致意見。探討同期行雙側(cè)膝關(guān)節(jié)表面置換(Total Knee Arthroplasty,TKA)時同時行髕骨置換對治療膝關(guān)節(jié)骨性關(guān)節(jié)炎(osteoarthritis,OA)的療效分析。方法選擇2013-02/2014-02青島大學(xué)附屬醫(yī)院關(guān)節(jié)外科收治的膝骨性關(guān)節(jié)炎行雙側(cè)膝關(guān)節(jié)置換術(shù)患者30例(60膝)�；颊呔炇鹬橥鈺�。將60膝不分左右隨機(jī)分為兩組,試驗(yàn)組30膝(左膝17例,右膝13例),術(shù)中行髕骨置換,對照組30膝(左膝13例,右膝17例),術(shù)中未置換髕骨。對兩組術(shù)后3月、6月及12月時膝關(guān)節(jié)功能(knee society score,KSS)評分、膝前痛評分、膝關(guān)節(jié)活動度和髕前彈響等進(jìn)行對比分析。結(jié)果置換組與未置換組患者術(shù)后各項(xiàng)指標(biāo)均較術(shù)前有明顯提高,術(shù)后3月、6月、12月膝關(guān)節(jié)功能評分、膝前痛VAS評分及術(shù)后3月、6月、12月膝關(guān)節(jié)活動度差異無顯著性意義(P0.05)。結(jié)論同期雙側(cè)全膝關(guān)節(jié)置換術(shù)行髕骨置換與不置換具有相同的短期療效。
[Abstract]:Objective Total knee arthroplasty (TKA) is one of the most effective and successful procedures in the treatment of all kinds of end-stage or severe knee diseases, but there is no agreement on whether or not to perform patellar replacement in total knee arthroplasty.Objective to investigate the therapeutic effect of patellar replacement in the treatment of osteoarthritis of knee joint with total Knee arthroplasty (TKAA) at the same time.Methods Thirty patients with bilateral knee arthroplasty underwent bilateral knee arthroplasty in the Department of Arteriology, Qingdao University affiliated Hospital, Qingdao University, 2013-02 / 2014-02.All patients signed informed consent.60 knees were randomly divided into two groups: 30 knees (17 left knee and 13 right knee) were treated with patellar replacement in the experimental group and 30 knees in the control group (13 left knee and 17 right knee without patella replacement).The knee society score, anterior knee pain score, knee motion and prepatellar bounce were compared between the two groups at 3 months, 6 months and 12 months after operation.Results the postoperative indexes in the replacement group and the non-replacement group were significantly higher than those in the preoperative group. There was no significant difference in the knee joint function score, the VAS score for anterior knee pain and the knee motion in the 3rd, 6th and 12th month postoperatively. There was no significant difference in the range of knee motion between 3 months, 6 months and 12 months after operation (P 0.05).Conclusion bilateral total knee arthroplasty has the same short-term effect as non-replacement.
