酒精對(duì)股骨頭局部微環(huán)境影響的機(jī)制研究進(jìn)展

【摘要】:背景:酒精性股骨頭壞死的發(fā)病機(jī)制目前尚不明確。目的:介紹股骨頭微環(huán)境的生理結(jié)構(gòu)和酒精致股骨頭微環(huán)境的病理變化,綜述酒精對(duì)股骨頭微環(huán)境影響的研究現(xiàn)狀。方法:應(yīng)用計(jì)算機(jī)檢索1990至2016年中國(guó)知網(wǎng)及PubMed數(shù)據(jù)庫(kù)關(guān)于酒精對(duì)股骨頭局部微環(huán)境影響的臨床及實(shí)驗(yàn)文獻(xiàn)報(bào)道,歸納綜述酒精性股骨頭壞死的病理過(guò)程。結(jié)果與結(jié)論:酒精性股骨頭壞死的發(fā)病機(jī)制目前有多種學(xué)說(shuō),但其本質(zhì)均為股骨頭骨組織的血液循環(huán)障礙,骨的營(yíng)養(yǎng)物質(zhì)和氧供減少,骨組織、細(xì)胞長(zhǎng)期缺氧而發(fā)生變性、壞死。大劑量酒精攝入可引起氧化-抗氧化系統(tǒng)失衡,嚴(yán)重影響股骨頭局部血運(yùn)環(huán)境。氧環(huán)境改變加速股骨頭壞死,骨髓間充質(zhì)干細(xì)胞成骨分化能力減弱,骨吸收與骨形成的平衡遭到破壞,最終加劇股骨頭的壞死與骨結(jié)構(gòu)塌陷。通過(guò)調(diào)控股骨頭微環(huán)境的改變有望為延緩或阻止早期股骨頭壞死病變進(jìn)程提供新的治療策略。
[Abstract]:Background: the pathogenesis of alcoholic femoral head necrosis is unclear. Objective: to introduce the physiological structure of the femoral head microenvironment and the pathological changes of the fine wine femoral head microenvironment. Methods: the present situation of the research on the effect of alcohol on the microenvironment of the femoral head was reviewed. Methods: the clinical and experimental reports on the effects of alcohol on the local microenvironment of the femoral head were retrieved by computer from 1990 to 2016 in China and PubMed database. Results and conclusion: there are many theories on the pathogenesis of alcoholic femoral head necrosis, but its essence is the blood circulation disturbance of femoral head tissue and the decrease of nutrition and oxygen supply of bone. Bone tissue, cells denatured and necrotized by hypoxia for a long time. High dose alcohol intake can cause imbalance of oxidation-antioxidation system, and seriously affect local blood delivery environment of femoral head. Changes of oxygen environment accelerate necrosis of femoral head. The osteogenic differentiation ability of bone marrow mesenchymal stem cells was weakened and the balance between bone resorption and bone formation was destroyed. Finally, the necrosis of the femoral head and the collapse of the bone structure are aggravated. By regulating the changes of the microenvironment of the femoral head, it is expected to provide a new treatment strategy for delaying or preventing the process of the early osteonecrosis of the femoral head.
