【摘要】：目的：本課題通過對比觀察,探討前路手術(shù)結(jié)合益氣活血通絡法治療頸椎病的臨床療效。方法：根據(jù)臨床癥狀和影像學表現(xiàn),將符合標準的62例頸椎病患者隨機分成試驗組與對照組,每組各31例,均行頸椎前路椎體次全切除+椎間盤摘除+鈦網(wǎng)植骨融合鋼板內(nèi)固定術(shù)。術(shù)后對照組常規(guī)治療,試驗組在常規(guī)治療基礎上,術(shù)后第3天予以服用益氣活血通絡法代表方加昧補陽還五湯(免煎制劑,每天1劑,連服15天)。通過觀察兩組手術(shù)操作時間,術(shù)中出血量,術(shù)前及術(shù)后1、3個月神經(jīng)功能和軸性癥狀,術(shù)后3個月植骨融合等情況,應用SPSS19.0統(tǒng)計學軟件對所得數(shù)據(jù)進行分析,對比各指標組內(nèi)和組間的差異,以P0.05表示差異有統(tǒng)計學意義。結(jié)果：術(shù)前神經(jīng)功能及軸性癥狀組間比較無明顯差異,術(shù)后1、3個月神經(jīng)功能組間比較,試驗組優(yōu)于對照組；術(shù)后1個月軸性癥狀組間比較試驗組優(yōu)于對照組,術(shù)后3個月軸性癥狀兩組之間差異無統(tǒng)計學意義。兩組病例在手術(shù)操作時間、術(shù)中出血量、術(shù)后3個月植骨融合情況等方面差異無統(tǒng)計學意義(P0.05)。結(jié)論：前路手術(shù)結(jié)合益氣活血通絡法治療頸椎病能夠促進神經(jīng)功能恢復,有效地改善術(shù)前軸性癥狀和降低術(shù)后軸性癥狀的發(fā)生率,提高手術(shù)效果,臨床優(yōu)良率達到100%,值得推廣。
[Abstract]:Objective: to explore the clinical effect of anterior approach combined with the method of supplementing qi and activating blood circulation and dredging collaterals in the treatment of cervical spondylosis by comparative observation. Methods: according to the clinical symptoms and imaging manifestations, 62 patients with cervical spondylosis were randomly divided into experimental group and control group with 31 cases in each group. On the basis of routine treatment, the experimental group was treated with Yiqi Huoxue Tongluo decoction on the third day after operation (no decoction, 1 dose per day, 15 days). By observing the operation time and the amount of blood loss during the operation, the experimental group was treated with Fubuyang Huanwu decoction. The neurological function and axonal symptoms before and 1, 3 months after operation, bone graft fusion at 3 months after operation and so on were analyzed by SPSS19.0 statistical software, and the differences among groups were compared. Results: there was no significant difference in preoperative nerve function and axonal symptoms between the two groups. After 1 month and 3 months after operation, the experimental group was better than the control group. One month after operation, there was no significant difference between the two groups in axial symptoms, but there was no significant difference between the two groups in three months after operation. The operation time and the amount of blood lost during the operation of the two groups were significantly higher than those in the control group, and there was no significant difference between the two groups. There was no significant difference in bone graft fusion in 3 months after operation (P 0.05). Conclusion: anterior approach combined with Yiqi Huoxue Tongluo method for cervical spondylosis can promote the recovery of nerve function. It can effectively improve the preoperative axial symptoms and reduce the incidence of postoperative axial symptoms, and improve the surgical effect. The clinical excellent and good rate is 100, which is worth popularizing.
