改良骨水泥推注套筒在經(jīng)皮椎體成形術(shù)中的應(yīng)用

【摘要】:目的比較改良骨水泥推注套筒(前端封閉,單側(cè)開口)與傳統(tǒng)直推骨水泥套筒在經(jīng)皮穿刺椎體成形術(shù)(PVP)中的臨床應(yīng)用。方法自2015年1月至2016年6月選取78例有PVP指征的單節(jié)骨質(zhì)疏松壓縮性骨折(OVCF),隨機(jī)分為觀察組與對照組(各39例),兩組均在DSA透視下實施PVP。對比兩組術(shù)中X線暴露劑量、手術(shù)時間、骨水泥滲漏情況、VAS評分(術(shù)前、術(shù)后3 d、術(shù)后6個月)、術(shù)后半年內(nèi)再骨折率及術(shù)后骨水泥分布情況。結(jié)果兩組均獲得隨訪6~12月,骨水泥滲漏情況:觀察組椎旁5例,對照組椎旁9例,椎前1例。兩組均未發(fā)生神經(jīng)及血管損傷,無肺栓塞等嚴(yán)重并發(fā)癥。兩組患者隨訪半年均未發(fā)生再骨折。兩組放射劑量及手術(shù)時間相比有統(tǒng)計學(xué)意義(P0.05);兩組骨水泥注入量及術(shù)前術(shù)后VAS評分相比無明顯差異(P0.05),兩組骨水泥分布比較無明顯差異(P0.05),觀察組骨水泥滲漏率低于對照組,有統(tǒng)計學(xué)意義(P0.05)。結(jié)論改良骨水泥推注套筒的使用可縮短手術(shù)時間,減少術(shù)中X線暴露及降低骨水泥滲漏率,臨床效果無差異。
[Abstract]:Objective to compare the modified bone cement injection sleeve. The clinical application of unilateral opening) and traditional direct thrust cement sleeve in percutaneous vertebroplasty. Methods from January 2015 to June 2016, 78 patients with single osteoporotic compression fracture with PVP indication were randomly divided into two groups. The observation group and the control group (39 cases each) were treated with PVP under DSA fluoroscopy. Time of operation, bone cement leakage and VAS score (3 days before operation, 3 days after operation, 6 months after operation, refracture rate within half a year after operation and the distribution of bone cement after operation). Results the follow up of the two groups was 6 ~ 12 months, the condition of bone cement leakage: 5 cases in the observation group, 5 cases in the observation group, 5 cases in the observation group, and 5 cases in the observation group. In the control group, 9 cases were paravertebral and 1 case was prevertebral. There were no serious complications such as pulmonary embolism. No refracture occurred in the two groups after half a year follow-up. There was no significant difference in radiation dose and operation time between the two groups (P 0.05), and there was no significant difference between the two groups in bone cement injection volume and VAS score before and after operation. There was no significant difference in the distribution of bone cement between the two groups. The leakage rate of bone cement in the observation group was lower than that in the control group. Conclusion the application of modified bone cement injection sleeve can shorten the operation time, reduce X-ray exposure during operation and reduce the rate of bone cement leakage. There is no difference in clinical effect.
