【摘要】：目的:比較吻合血管腓骨皮瓣移植法與萬古霉素硫酸鈣填塞法治療慢性脛骨骨髓炎并骨缺損的臨床療效。方法:我院35例慢性脛骨骨髓炎并骨缺損患者,分別接受吻合血管的腓骨皮瓣移植法(A組21例)或萬古霉素硫酸鈣填塞法(B組14例),比較兩組感染控制率及Enneking系統(tǒng)評分,同時檢測術前與術后2周、術后1及3個月的CRP及PCT水平。結果:兩組患者術后CRP及PCT均能控制,術前與術后2周、術后1及3個月比較,差異有統(tǒng)計學意義。在感染控制率上兩組對比差異無統(tǒng)計學意義(P0.05),術后Enneking評分上相比差異有統(tǒng)計學意義(P0.05)。結論:兩種方法臨床療效均滿意,應視患者感染的程度和骨缺損的長短選擇合適的治療辦法,有時聯(lián)用兩種治療方法更值得推薦。
[Abstract]:Objective: to compare the clinical effects of vascularized fibula flap transplantation and vancomycin calcium sulfate tamponade in the treatment of chronic tibial osteomyelitis with bone defect. Fibula flap graft was performed with vascularized fibula flap (n = 21) or vancomycin calcium sulfate tamponade (group B, n = 14). The infection control rate and Enneking system score were compared between the two groups. Results: the levels of CRP and PCT in both groups were controlled 1 and 3 months after operation, and were compared between preoperative and postoperative 2 weeks, 1 and 3 months after operation. The difference was statistically significant. There was no significant difference in infection control rate between the two groups (P 0.05), but there was a significant difference in Enneking score after operation. Conclusion: the clinical efficacy of the two methods is satisfactory. The appropriate treatment should be selected depending on the degree of infection and the length of bone defect.
