【摘要】：明確終板在椎間盤的生理和退變過程中起的作用,對相關(guān)知識進(jìn)行綜述。查閱近10年來關(guān)于終板與椎間盤退變的文獻(xiàn),整合相關(guān)知識,圍繞終板及其退變過程對椎間盤退變作出新的解釋。結(jié)果發(fā)現(xiàn)終板可以分為骨性終板和軟骨終板,位于椎體和椎間盤之間,起著保護(hù)椎體和椎間盤、營養(yǎng)椎間盤的作用。由于基因、年齡、力學(xué)負(fù)載、損傷等眾多因素導(dǎo)致終板的保護(hù)作用下降甚至喪失,椎間盤的主要營養(yǎng)通路破壞,終板發(fā)生退變,軟骨終板鈣化,邊緣不規(guī)則,進(jìn)而出現(xiàn)椎間隙狹窄、骨贅形成,影像學(xué)上出現(xiàn)Modic改變,臨床上出現(xiàn)腰痛的癥狀。終板是椎體-終板-椎間盤系統(tǒng)的重要組成部分,無論對椎體還是椎間盤都有著至關(guān)重要的作用。終板退變可能是椎間盤退變的前驅(qū)變化,研究終板退變的機(jī)制將會對闡明椎間盤退變的機(jī)制有重要的意義。
[Abstract]:To clarify the role of endplate in the physiology and degeneration of intervertebral disc, and to review the relevant knowledge of endplate and intervertebral disc degeneration in the past 10 years. A new explanation of intervertebral disc degeneration is given around the endplate and its degenerative process. The results show that the endplate can be divided into bony endplate and cartilage endplate, located between the vertebral body and the intervertebral disc, which protects the vertebral body and the intervertebral disc. The role of vegetative disc. Due to many factors such as gene, age, mechanical load, injury and so on, the protective effect of endplate is decreased or even lost, the main nutritional pathway of disc is destroyed, endplate degeneration, cartilage endplate calcification, etc. The margin is irregular, resulting in narrow intervertebral space, osteophyte formation, imaging changes in Modic, and clinical symptoms of low back pain. The endplate is an important part of the vertebro-endplate intervertebral disc system. Degenerative endplate degeneration may be the precursor of disc degeneration, and studying the mechanism of endplate degeneration will be of great significance to clarify the mechanism of intervertebral disc degeneration.
