【摘要】：背景肌內(nèi)效貼是最為風(fēng)行的軟組織貼扎技術(shù)之一,在運(yùn)動醫(yī)學(xué)、康復(fù)醫(yī)學(xué)領(lǐng)域常用于改善各類疼痛、提高運(yùn)動能力,并可持續(xù)起效,起到所謂“將治療師的手帶回家”的作用。關(guān)節(jié)松動技術(shù)是現(xiàn)代康復(fù)治療的基本技術(shù),可針對下腰痛肌肉、筋膜及關(guān)節(jié)進(jìn)行治療,以糾正腰椎小關(guān)節(jié)紊亂和調(diào)整脊柱生物力學(xué)改變,臨床應(yīng)用方面有一定的療效,但就其治療下腰痛臨床療效的維持、隨訪等方面仍需展開科學(xué)實證研究。將肌內(nèi)效貼與關(guān)節(jié)松動術(shù)相結(jié)合,可給非特異性下腰痛綜合康復(fù)方案提供新思路,有進(jìn)一步探討的價值和必要。研究目的將肌內(nèi)效貼與關(guān)節(jié)松動術(shù)相結(jié)合,應(yīng)用于慢性非特異性下腰痛老年患者,并通過對患者腰背肌功能、疼痛情況及患者治療的滿意度等進(jìn)行觀察,探討二者結(jié)合對非特異性下腰痛的治療作用,并對其相關(guān)可能作用機(jī)制進(jìn)行初步探索,以期為臨床治療與后續(xù)研究提供一定的證據(jù)支持和參考。研究方法將符合標(biāo)準(zhǔn)的老年男性非特異性下腰痛患者,隨機(jī)分為關(guān)節(jié)松動術(shù)結(jié)合肌內(nèi)效+常規(guī)理療組(觀察組)18例及常規(guī)理療組(對照組)13例。在入選時及治療后1周,分別對兩組患者進(jìn)行腰背肌屈伸肌群峰力矩(peak torque,PT)、疼痛評分(visual analogue scale,VAS)及Oswestry腰痛失能問卷調(diào)查量表(Oswestry PainDisability Questionnaire,OPDQ)的評定,比較兩組患者上述指標(biāo)的差別。研究結(jié)果1.患者的一般基線情況具有可比性,治療前各項評分兩組組間沒有明顯差異;2.治療1周后組間比較,觀察組屈、伸肌群PT較對照組均有統(tǒng)計學(xué)意義顯著性差異(P0.05);3.治療1周后組間比較,組間比較觀察組VAS、OPDQ較對照組均有統(tǒng)計學(xué)意義極顯著性差異(P0.01);4.患者治療前后疼痛增加程度與屈伸肌群峰力矩呈顯著負(fù)相關(guān)(P0.01)。研究結(jié)論肌內(nèi)效貼配合關(guān)節(jié)松動術(shù)可更顯著改善老年男性非特異性下腰痛患者近期癥狀及功能障礙情況。
[Abstract]:Background intramuscular patch is one of the most popular soft tissue sticking techniques. In sports medicine, rehabilitation medicine is often used to improve all kinds of pain, improve motor ability, and can continue to work. The technique of joint loosening is the basic technique of modern rehabilitation. It can be used to treat the muscles, fascia and joints of lower back pain. In order to correct the disorder of lumbar facet joint and adjust the biomechanical change of spine, it has certain curative effect in clinical application, but the maintenance of clinical curative effect in treating low back pain, The combination of intramuscular patch and joint loosening can provide a new idea for the comprehensive rehabilitation of non-specific low back pain. The purpose of this study was to apply intramuscular patch and joint loosening to elderly patients with chronic nonspecific low back pain, and to evaluate the function of lumbar and dorsal muscles in patients with chronic nonspecific low back pain. To investigate the therapeutic effect of the two methods on non-specific low back pain, and to explore the possible mechanism of the treatment. In order to provide some evidence and reference for clinical treatment and follow-up study, the research method will meet the standard of elderly male patients with non-specific low back pain, They were randomly divided into two groups: the control group (n = 13) and the control group (n = 18). The patients in the two groups were assessed with peak torque of the lumbar dorsalis flexion and extensor muscle, visual analogue scale and Oswestry low back pain disability scale (Oswestry PainDisability Questionnaive OPDQ). Comparison of the above indexes between the two groups. 1. The general baseline of the patients was comparable, and there was no significant difference between the two groups before treatment. 2. After one week of treatment, the comparison between the two groups, the observation group flexion, The PT of extensor muscle group was significantly higher than that of control group (P 0.05). Compared with the control group, VASA OPDQ in the observation group was significantly different from that in the control group. There was a significant negative correlation between the increase of pain and the peak moment of flexion and extensor muscle before and after treatment. Conclusion the intramuscular patch combined with joint loosening is more effective than that of the control group. To improve the recent symptoms and dysfunction in elderly men with non-specific low back pain.
