微等離子體射頻技術(shù)對(duì)NB-UVB誘導(dǎo)豚鼠皮膚色素沉著的影響

【摘要】:目的:微等離子體射頻技術(shù)(micro-plasma radio-frequency technology)作為一種新技術(shù),近些年來在改善皮膚細(xì)紋、粗糙、松弛、瘢痕及色素沉著等治療中顯示出良好的應(yīng)用前景。臨床上,應(yīng)用微等離子體射頻技術(shù)治療瘢痕,從療效、安全性、恢復(fù)時(shí)間的總體評(píng)估上,有著顯著的優(yōu)越性。在微等離子體射頻技術(shù)的臨床應(yīng)用中,被證實(shí)能顯著改善燒傷后瘢痕及皮膚色素沉著。本文通過研究應(yīng)用微等離子體射頻技術(shù)治療窄波中波紫外線(narrow band ultraviolet B,NB-UVB)誘導(dǎo)下豚鼠皮膚色素沉著動(dòng)物模型,觀測與豚鼠皮膚色素沉著相關(guān)的酪氨酸酶活性的黑色素細(xì)胞數(shù)量、黑素顆粒、黑色素細(xì)胞功能活性的表達(dá)指標(biāo),為指導(dǎo)臨床治療瘢痕及色素沉著性疾病提供理論依據(jù)。1建立動(dòng)物模型:選擇10只健康豚鼠(普通級(jí)),雌性5只,雄性5只,體重0.25~0.35kg,7~10周齡,對(duì)豚鼠棕黃色毛發(fā)皮膚區(qū)域脫毛后行NB-UVB照射,照射劑量:900 m J/cm~2,1次/周,共3次。統(tǒng)計(jì)NB-UVB照射前后豚鼠皮膚酪氨酸酶活性的黑色素細(xì)胞數(shù)目、黑素顆粒面積占表皮面積百分比、c-kit蛋白陽性細(xì)胞數(shù)目。模型皮膚較正常皮膚各項(xiàng)指標(biāo)均明顯增加,差異有統(tǒng)計(jì)學(xué)意義(P0.05),可判斷為造模成功。2造模成功區(qū)域隨機(jī)分為4組,以不同能量參數(shù)的微等離子體射頻技術(shù)進(jìn)行治療:A組:對(duì)照組。B、C、D組:治療組,治療參數(shù)為作用時(shí)間0.6s,輸出功率依次為10Watt、20Watt、30Watt的微等離子體射頻技術(shù)照射治療。分別在治療前和完成治療后2、4、6周,取材進(jìn)行黑色素細(xì)胞染色(表皮分離-多巴染色法)、黑素顆粒染色(Masson—Fontana法)、以及免疫組織化學(xué)染色(c-kit蛋白),觀察酪氨酸酶活性的黑色素細(xì)胞數(shù)目、黑素顆粒面積占表皮面積百分比、黑色素細(xì)胞活性的改變。3應(yīng)用SPSS13.0統(tǒng)計(jì)學(xué)軟件對(duì)酪氨酸酶活性的黑色素細(xì)胞數(shù)目、黑素顆粒所占表皮面積百分比、c-kit蛋白陽性細(xì)胞數(shù)目進(jìn)行統(tǒng)計(jì)學(xué)分析。NB-UVB照射前后數(shù)據(jù)采用配對(duì)t檢驗(yàn)進(jìn)行比較;微等離子體射頻技術(shù)治療結(jié)束后2、4、6周各處理組間數(shù)據(jù)比較,符合正態(tài)且方差齊的數(shù)據(jù)應(yīng)用區(qū)組方差分析,正態(tài)且方差不齊或非正態(tài)的數(shù)據(jù)采用區(qū)組秩和檢驗(yàn)。對(duì)各時(shí)間點(diǎn)酪氨酸酶活性的黑色素細(xì)胞數(shù)目、黑素顆粒所占表皮面積百分比進(jìn)行重復(fù)測量的方差分析。以(P0.05)作為差異顯著性的判斷標(biāo)準(zhǔn)。分析探討微等離子體射頻技術(shù)對(duì)NB-UVB誘導(dǎo)豚鼠皮膚色素沉著的影響。結(jié)果:微等離子體射頻技術(shù)治療后色素沉著觀測指標(biāo)的改變1酪氨酸酶活性的黑色素細(xì)胞數(shù)目結(jié)果統(tǒng)計(jì)分析:治療結(jié)束后2、4周:B、C、D組較A組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);BCD,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療結(jié)束后6周:B、C、D組較A組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);B與C組相比差異無統(tǒng)計(jì)學(xué)意義(P0.05),D組較B、C組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。各時(shí)間點(diǎn)重復(fù)測量比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。時(shí)間因素、分組因素主效應(yīng)均存在,二者交互作用也存在。2黑素顆粒所占表皮面積百分比變化結(jié)果統(tǒng)計(jì)分析:治療結(jié)束后2周,B、C、D組較A組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);B與C無差異;D組較B、C組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療結(jié)束后4周,B、C、D組較A組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);C、D組較B組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);C與D組相比差異無統(tǒng)計(jì)學(xué)意義(P0.05)。治療結(jié)束后6周,僅D組低于A組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);D組較B組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05);其余組間相比,差異無統(tǒng)計(jì)學(xué)意義(P0.05)。各時(shí)間點(diǎn)重復(fù)測量比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。時(shí)間因素、分組因素主效應(yīng)均存在,二者交互作用也存在。3 c-kit蛋白陽性細(xì)胞數(shù)目結(jié)果統(tǒng)計(jì)分析:治療結(jié)束后2周,B、C、D組較A組低,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。治療結(jié)束后4、6周,未發(fā)現(xiàn)c-kit蛋白陽性細(xì)胞存在。結(jié)論:微等離子體射頻技術(shù)可加速豚鼠皮膚色素沉著的消退,對(duì)改善NB-UVB照射后豚鼠皮膚色素沉著的動(dòng)物模型有效。
[Abstract]:Purpose: micro plasma radio frequency technology (micro-plasma radio-frequency technology) is a new technology in recent years, improve the skin wrinkles, rough, loose, and shows a good prospect in the treatment of scar and pigmentation. Clinically, application of micro plasma radio frequency technology for the treatment of scar, from the curative effect, safety evaluation, overall recovery time, there are significant advantages. In the clinical application of micro plasma radio frequency technology, has been shown to significantly improve the burn scar and skin pigmentation. Through the study on the application of micro plasma radiofrequency for narrow-band UVB (narrow band ultraviolet B, NB-UVB) induced skin pigmentation of guinea pig animal model tyrosinase activity, observation and skin pigmentation related to the number of melanocytes, melanin granules, expression index of melanoma cell function activity .1, provide a theoretical basis for the establishment of animal model for clinical treatment of scar and pigmentation disorders: choose 10 healthy guinea pigs (general), 5 female, 5 male, weight 0.25~0.35kg, 7~10 weeks old, brown hair of guinea pig skin area hair removal NB-UVB after irradiation, irradiation dose: 900 m / J/cm~2,1 week, a total of 3 times.
