【摘要】：目的:探討應(yīng)用彎角裝置單側(cè)穿刺行經(jīng)皮椎體后凸成形術(shù)(percutaneous vertebroplasty,PVP)治療骨質(zhì)疏松性胸腰椎壓縮骨折(osteoporotic vertebral compression fractures,OVCFs)的臨床效果。方法:回顧性分析我院2015年1月~2016年1月應(yīng)用彎角裝置行單側(cè)穿刺PVP治療的47例OVCFs患者的臨床資料,男14例,女33例;年齡53~89歲(66.4±6.5歲)。36例為單節(jié)段椎體骨折,11例為雙節(jié)段椎體骨折;骨折椎體:T9 7節(jié),T10 7節(jié),T11 9節(jié),T12 13節(jié),L1 12節(jié),L2 9節(jié),L3 1節(jié)。記錄術(shù)中透視時(shí)間、骨水泥用量,采用VAS評(píng)分、ODI評(píng)估手術(shù)前后疼痛程度及腰椎功能情況;測量計(jì)算手術(shù)前后傷椎相對(duì)高度,觀察局部后凸Cobb角恢復(fù)情況;觀察術(shù)中骨水泥滲漏(靜脈滲漏及椎體周圍滲漏)及其他并發(fā)癥的發(fā)生情況。結(jié)果:術(shù)中透視時(shí)間1.6±0.3min;骨水泥用量6.7±1.2ml,骨水泥分布均勻;11例患者術(shù)中出現(xiàn)骨水泥滲漏,骨水泥滲漏率為23.4%。術(shù)后隨訪3~12個(gè)月(6.5±1.3個(gè)月),術(shù)前、術(shù)后2d和末次隨訪時(shí)的VAS評(píng)分、ODI、傷椎相對(duì)高度及局部后凸Cobb角分別為:7.6±1.3分、(71.4±3.2)%、0.48±0.21、15.5°±4.2°;2.2±1.0分、(27.2±2.6)%、0.82±0.17、7.2°±2.8°;1.7±0.7分、(26.5±2.7)%、0.80±0.15、7.5°±3.7°;術(shù)后2d和末次隨訪時(shí)的VAS評(píng)分、ODI和局部后凸Cobb角均較術(shù)前明顯改善,傷椎相對(duì)高度較術(shù)前均明顯恢復(fù),差異均有統(tǒng)計(jì)學(xué)意義(P0.05);術(shù)后2d與末次隨訪時(shí)比較差異均無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:應(yīng)用彎角裝置行單側(cè)穿刺PVP治療OVCFs可使骨水泥分布均勻,安全有效,臨床效果滿意。
[Abstract]:Objective: To investigate the application of corner device of unilateral puncture percutaneous kyphoplasty (percutaneous vertebroplasty, PVP) for the treatment of osteoporotic thoracolumbar vertebral compression fracture (osteoporotic vertebral compression fractures, OVCFs) clinical effect.
