富血小板血漿治療椎間盤退變:從基礎研究到臨床應用

【摘要】:背景:椎間盤退變是下腰痛的常見原因,椎間盤內(nèi)的微環(huán)境和細胞環(huán)境發(fā)生變化是誘發(fā)椎間盤退變的重要因素,但目前仍無確鑿證據(jù)表明退變發(fā)生的機制。目的:針對當前富血小板血漿治療椎間盤退變的研究概況做一綜述。方法:設計綜述的撰寫結(jié)構(gòu),在萬方數(shù)據(jù)庫和Pub Med數(shù)據(jù)庫中分別輸入中、英文檢索詞,分析所得文獻的摘要及內(nèi)容,通過納入和排除得到相關文獻。結(jié)果與結(jié)論:盡管手術(shù)治療中晚期椎間盤退變有顯著療效,但對于早期退變,仍沒有較好的保守治療方法。富血小板血漿可以促進組織愈合和再生,已被諸多學者用于椎間盤退變模型的治療中。細胞學實驗及動物實驗證明,富血小板血漿可以有效延緩甚至逆轉(zhuǎn)椎間盤早期退變;臨床試驗表明,下腰痛患者接受富血小板血漿治療后疼痛有所改善,將富血小板血漿和其他生物學治療結(jié)合,能夠顯著改善椎間盤退變。目前的大多數(shù)實驗都局限于細胞學和動物學研究,臨床治療的報道少之又少,加之富血小板血漿的提取方法、注射劑量以及最佳注射時間都無統(tǒng)一標準,而對富血小板血漿治療的不良反應也知之甚少,故富血小板血漿治療椎間盤退變的療效有待于進一步驗證。
[Abstract]:Background: disc degeneration is a common cause of low back pain. The changes of microenvironment and cellular environment in intervertebral disc are important factors to induce disc degeneration. But there is still no conclusive evidence to indicate the mechanism of degeneration. Objective: to review the current research of platelet-rich plasma in the treatment of disc degeneration. Input Chinese and English key words in Wanfang database and Pub Med database, analyze the abstracts and contents of the literature, Results and conclusions: although the surgical treatment of intervertebral disc degeneration in the middle and late stage has a significant effect, it is effective in the early stage of degeneration. Platelet-rich plasma can promote tissue healing and regeneration, and has been used by many scholars in the treatment of intervertebral disc degeneration model. Cytological and animal experiments have proved that platelet rich plasma can promote tissue healing and regeneration. Platelet rich plasma can effectively delay or even reverse the early degeneration of intervertebral disc. Clinical trials show that the pain of patients with low back pain after treatment with platelet-rich plasma can be improved by combining platelet-rich plasma with other biological treatments. Most of the current experiments are limited to cytological and zoological studies, with few reports of clinical treatment and methods of extracting platelet-rich plasma. There is no uniform standard for injection dose and optimal injection time, and little is known about the adverse reactions of platelet-rich plasma therapy, so the efficacy of platelet-rich plasma in treating disc degeneration needs further verification.
