【摘要】：目的探討股骨頭壞死(ONFH)的組織病理學(xué)特點(diǎn),為全面認(rèn)識(shí)該疾病和臨床防治提供依據(jù)。方法收集本院2011—2012年間手術(shù)切除ONFH送檢標(biāo)本20例,復(fù)習(xí)臨床資料,全面閱讀切片,總結(jié)分析。結(jié)果 20例中發(fā)生于右側(cè)11例,左側(cè)7例,雙側(cè)2例。主要癥狀為髂關(guān)節(jié)疼痛,活動(dòng)受限。平均病史8.2年。病理學(xué)檢查:股骨頭標(biāo)本以變形為主,關(guān)節(jié)面粗糙、凹凸不平,骺軟骨板厚薄不均或斷裂、分離、脫落;剖面骨質(zhì)實(shí)變,呈暗紅或雜色,失去正常蜂窩結(jié)構(gòu);組織學(xué)改變分3個(gè)層次,即軟骨退行性改變、死骨成分和間質(zhì)反應(yīng)性增生。由于病變進(jìn)展期不同,損害程度差異,使得病變組織結(jié)構(gòu)復(fù)雜多樣,相互交織,病變不僅能破壞正常組織結(jié)構(gòu),亦可破壞關(guān)節(jié)累及周圍滑膜組織。結(jié)論 ONFH是常見(jiàn)病,臨床及影像學(xué)即可明確診斷,病理組織學(xué)的深入觀察可為早期防治提供理論依據(jù)。
[Abstract]:Objective to investigate the histopathological features of osteonecrosis of the femoral head (ONFH), and to provide evidence for comprehensive understanding of the disease and its clinical prevention and treatment. Results among the 20 cases, 11 cases occurred on the right side, 7 cases on the left side and 2 cases on the bilateral side. The main symptoms were pain of iliac joint and limited movement. The average history was 8.2 years. Pathological examination showed that the specimen of femoral head was mainly deformed and the articular surface was rough. Uneven, uneven thickness or rupture of epiphyseal cartilage plate, separation and shedding; section bone consolidation, dark red or miscellaneous color, loss of normal honeycomb structure; histological changes are divided into three levels, that is, cartilage degenerative changes, Due to the difference of pathological progression and damage degree, the pathological tissue structure is complicated and varied, intertwined with each other, and the lesion can not only destroy the normal tissue structure, but also damage the normal tissue structure. Conclusion ONFH is a common disease which can be diagnosed by clinic and imaging. The further observation of histopathology can provide theoretical basis for early prevention and treatment.
