【摘要】：目的:收集近年來本院治療的兒童Perthes病病例,比較Salter骨盆截骨及股骨內(nèi)翻截骨術(shù)治療兒童Perthes病的療效。方法:2004~2012年有29例Perthes病住院病例,分2組進行比較。A組(Salter骨盆截骨組)17例;B組(股骨內(nèi)翻截骨組)12例。根據(jù)臨床結(jié)果及X線Stulberg標準進行療效評價。結(jié)果:經(jīng)3~9年隨訪,2組的臨床表現(xiàn)及功能上無明顯差異;按Stulberg標準優(yōu)良率分別為:A組65%B組58%,差別無統(tǒng)計學意義;在X線片表現(xiàn)上,包括患側(cè)股骨頸干角、關(guān)節(jié)轉(zhuǎn)子間距等指標上,Salter骨盆截骨組與健側(cè)差異較小;兩組髖臼的覆蓋無明顯差異。結(jié)論:Salter骨盆截骨和股骨近端內(nèi)翻截骨兩種包容手術(shù)治療Perthes病的臨床療效之間沒有明顯差異,股骨頭的形態(tài)以及髖關(guān)節(jié)協(xié)調(diào)性上均沒有顯著差異。然而,Salter骨盆截骨組的頸干角的大小,關(guān)節(jié)轉(zhuǎn)子間距更接近正常髖關(guān)節(jié)。我們認為,Salter骨盆截骨在治療Perthes時的優(yōu)勢在于減少術(shù)后殘余畸形,如髖關(guān)節(jié)內(nèi)翻,大粗隆過度生長。
[Abstract]:Objective: to compare the efficacy of Salter pelvic osteotomy and femoral varus osteotomy in the treatment of children with Perthes's disease. Methods: from 2004 to 2012, 29 cases of Perthes's disease were hospitalized. Two groups were divided into two groups. There were 17 cases of Salter pelvis osteotomy in group A and 12 cases in group B (femoral varus osteotomy group, 12 cases). The curative effect was evaluated according to the clinical results and X-ray Stulberg standard. Results: there was no significant difference in clinical manifestation and function between the two groups after 3 ~ 9 years follow-up. According to the standard of Stulberg, the excellent and good rates were 65% in group A and 58 in group B. there was no significant difference in X-ray findings, including the angle of femoral neck trunk of the affected side, the distance between the femoral neck and the rotors of the affected side, and the difference between the osteotomy group of pelvis and the healthy side. There was no significant difference in acetabular coverage between the two groups. Conclusion there is no significant difference between the two types of inclusive surgery in the treatment of Perthes's disease. There was no significant difference in the shape of the femoral head and the coordination of the hip joint. However, the size of the neck shaft angle in the Salter pelvic osteotomy group, We believe that the advantage of Salter pelvis osteotomy in the treatment of Perthes lies in the reduction of postoperative residual deformities such as hip varus and overgrowth of the great trochanter.
