【摘要】：目的:對(duì)比研究Acutrak螺釘與空心螺釘在治療Lisfranc關(guān)節(jié)損傷方面的臨床療效,旨在為骨科醫(yī)師在Lisfranc關(guān)節(jié)損傷的臨床施治過(guò)程中提供專業(yè)的診治意見(jiàn)和經(jīng)驗(yàn)交流。方法:采用多中心回顧性研究的方式,收集2010年9月至2016年8月期間在新疆烏魯木齊市地區(qū)某三所醫(yī)院采用Acutrak螺釘或空心螺釘手術(shù)治療的Lisfranc關(guān)節(jié)損傷患者28例,根據(jù)其手術(shù)治療所采用耗材種類的不同分為使用Acutrak螺釘?shù)挠^察組(n=10)和使用空心螺釘?shù)膶?duì)照組(n=18)。記錄根據(jù)視覺(jué)模擬評(píng)分系統(tǒng)(VAS)和美國(guó)足踝外科評(píng)分系統(tǒng)(AOFAS)末次隨訪所獲評(píng)分用以對(duì)術(shù)后治療效果進(jìn)行評(píng)價(jià),同時(shí)參與的評(píng)價(jià)指標(biāo)還包括手術(shù)時(shí)間、并發(fā)癥、下地完全負(fù)重時(shí)間、骨折愈合時(shí)間和螺釘平均價(jià)格,最后利用spss軟件對(duì)兩組情況進(jìn)行對(duì)比分析。結(jié)果:本研究所涉及28例患者均獲得隨訪,平均隨訪時(shí)間為14.8±21.1個(gè)月。觀察組與對(duì)照組在手術(shù)時(shí)間、骨折愈合時(shí)間、術(shù)后VAS評(píng)分及術(shù)后并發(fā)癥的發(fā)病率方面的比較,均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05);觀察組在下地完全負(fù)重時(shí)間早于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P0.05);根據(jù)AOFAS評(píng)分進(jìn)行評(píng)價(jià),觀察組的優(yōu)良率雖高于對(duì)照組,但組間比較無(wú)統(tǒng)計(jì)學(xué)意義;觀察組與對(duì)照組間螺釘平均價(jià)格的比較,差異具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:Acutrak螺釘與空心螺釘在治療Lisfranc關(guān)節(jié)損傷方面療效相當(dāng),均為十分有效治療Lisfranc關(guān)節(jié)損傷的耗材種類,但考慮到治療成本的因素,故謹(jǐn)慎認(rèn)為在兩者螺釘間做比較選擇時(shí),不建議優(yōu)先使用Acutrak螺釘治療Lisfranc關(guān)節(jié)損傷。
[Abstract]:Objective: to compare the clinical effect of Acutrak screw and hollow screw in the treatment of Lisfranc joint injury. The purpose of this study was to provide orthopedic physicians with professional advice and exchange of experience in the clinical treatment of Lisfranc joint injury. From September 2010 to August 2016, 28 patients with Lisfranc joint injuries treated with Acutrak screws or hollow screws in three hospitals in Urumqi, Xinjiang, were collected. According to the type of consumables used in the surgical treatment, they were divided into two groups: the observation group using Acutrak screws (n = 10) and the control group with hollow screws (n = 18). The last follow-up was recorded according to the visual analogue scoring system (VAS) and the United States foot and ankle surgery scoring system (AFS). The score was used to evaluate the effect of the treatment. They also included surgery time, complications, total weight loading time, fracture healing time and average screw price. Results: 28 patients involved in this study were followed up for an average of 14.8 鹵21.1 months. The operative time and fracture healing time of the observation group and the control group were compared. There was no significant difference in the postoperative VAS score and the incidence of postoperative complications (P 0.05), the time of full weight loading in the observation group was earlier than that in the control group, the difference was statistically significant (P 0.05), and the evaluation was made according to the AOFAS score. Although the excellent and good rate of the observation group was higher than that of the control group, there was no statistical significance between the observation group and the control group. The difference was statistically significant (P 0.05). Conclusion: the ratio of Acutrak screw to hollow screw is equivalent in the treatment of Lisfranc joint injury, both of them are very effective in the treatment of Lisfranc joint injury, but considering the cost of treatment, Therefore, it is prudent to suggest that Acutrak screw should not be used in the treatment of Lisfranc joint injury.
