輕量型網(wǎng)片應(yīng)用于Lichtenstein疝修補(bǔ)術(shù)長期療效的系統(tǒng)評價(jià)和Meta分析

【摘要】:目的評價(jià)Lichtenstein修補(bǔ)術(shù)中輕量型網(wǎng)片與重量型網(wǎng)片的遠(yuǎn)期臨床療效及安全性。方法計(jì)算機(jī)檢索PubMed、EMBASE、The Cochrane Library、Web of Science、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫、相關(guān)期刊論文、萬方數(shù)字化期刊群和維普中文科技期刊數(shù)據(jù)庫,檢索時(shí)間均從建庫至2013年11月,同時(shí)追溯納入文獻(xiàn)的參考文獻(xiàn)。由兩名評價(jià)者按照納入與排除標(biāo)準(zhǔn)以及Cochrane協(xié)作網(wǎng)推薦的方法獨(dú)立篩選文獻(xiàn)、提取資料并評價(jià)納入研究的方法學(xué)質(zhì)量后,采用RevMan 5.2.5軟件進(jìn)行分析。結(jié)果共納入15個(gè)隨機(jī)對照試驗(yàn),包含3390例患者。分析結(jié)果顯示:與重量型網(wǎng)片相比,輕量型網(wǎng)片可減輕術(shù)后近期(≤12個(gè)月)慢性疼痛[RR=0.67,95%CI(0.53-0.83),P=0.0003]、減少術(shù)后異物感[RR=0.69,95%CI (0.56-0.85),P=0.0005];而在術(shù)后遠(yuǎn)期(12個(gè)月)疼痛[RR=0.93,95%CI(0.72-1.19), P=0.56]、復(fù)發(fā)[RR=1.21,95%CI (0.77-1.90), P=0.41]和睪丸萎縮[RR=1.90,95%CI(0.58-6.28),P=0.29]的發(fā)生率兩者比較差異無統(tǒng)計(jì)學(xué)意義。結(jié)論與重量型網(wǎng)片相比,輕量型網(wǎng)片可降低術(shù)后短期慢性疼痛和減少異物感,但并未增加復(fù)發(fā),因此在腹股溝疝修補(bǔ)術(shù)中,可以將輕量型網(wǎng)片作為首選。
[Abstract]:Objective to evaluate the long-term clinical efficacy and safety of light and weight mesh during Lichtenstein repair. Methods the Cochrane Library of Science, the Chinese Biomedical Literature Database and the full text Database of Chinese Journal were searched by computer. The search time of Wanfang Digital Journals Group and Weipu Chinese Science and Technology Journals Database is from the establishment of the database to November 2013, At the same time, two reviewers independently sifted the literature according to the inclusion and exclusion criteria and the method recommended by the Cochrane collaboration Network, extracted the data and evaluated the methodological quality of the study.
