【摘要】：目的探討胰十二指腸切除術(shù)中細(xì)口徑膽腸吻合使用改良的T管支撐引流的安全性和有效性。方法回顧性分析2009年8月至2014年8月第二軍醫(yī)大學(xué)附屬長海醫(yī)院胰腺外科行胰十二指腸切除術(shù)的膽管直徑8 mm、胰管直徑正常的166例病人資料,其中81例病人采用改良的T管支撐引流膽腸吻合(改良組),85例病人采用直接膽腸吻合(常規(guī)組),比較分析兩組膽瘺、胰瘺發(fā)生率及其他手術(shù)相關(guān)指標(biāo)的差異。結(jié)果改良組病人在術(shù)后胰瘺、膽瘺的發(fā)生率,術(shù)中出血量及住院時間,術(shù)后膽腸吻合口狹窄發(fā)生率均低于常規(guī)組(P均0.05)。結(jié)論改良的T管支撐引流膽腸吻合操作簡便,在降低胰瘺、膽瘺發(fā)生率方面有優(yōu)勢,同時存在術(shù)中出血少,住院時間短及術(shù)后膽腸吻合口狹窄發(fā)生率低等優(yōu)點,是胰十二指腸切除術(shù)中膽腸吻合的一種有效的改良方案,值得臨床推廣應(yīng)用。
[Abstract]:Objective to investigate the safety and efficacy of modified T-tube support drainage in pancreaticoduodenectomy with small caliber choledochojejunostomy. Methods from August 2009 to August 2014, the pancreas of Changhai Hospital affiliated to the second military Medical University was retrospectively analyzed. The diameter of bile duct was 8 mm and the diameter of pancreatic duct was normal in 166 patients who underwent pancreaticoduodenectomy. Among them, 81 patients were treated with modified T-tube drainage and cholangiojejunostomy. 85 patients in the modified group were treated with direct choledochojejunostomy. Results the incidence of pancreatic fistula, biliary fistula, intraoperative bleeding and hospital stay were significantly different in the modified group. The incidence of biliary anastomotic stenosis after operation was lower than that in the routine group (P < 0.05). Conclusion the modified T-tube supported drainage choledochojejunostomy is easy to operate and has advantages in reducing the incidence of pancreatic fistula and biliary fistula, at the same time, there is less bleeding during operation. The advantages of short hospital stay and low incidence of cholangiojejunostomy stenosis are an effective and improved method for cholangiojejunostomy in pancreaticoduodenectomy.
