【摘要】：目的探討目標(biāo)導(dǎo)向液體治療對(duì)腹腔鏡卵巢癌根治術(shù)患者肝腎功能的影響。方法選取華北理工大學(xué)附屬醫(yī)院2013年2月至2015年2月腹腔鏡卵巢癌根治術(shù)患者142例,分為觀察組和對(duì)照組,每組71例。對(duì)照組實(shí)施傳統(tǒng)補(bǔ)液方案,觀察組實(shí)施目標(biāo)導(dǎo)向液體治療。記錄兩組手術(shù)時(shí)間、術(shù)中出血量、術(shù)后排氣時(shí)間、術(shù)后進(jìn)食時(shí)間、術(shù)后住院時(shí)間、體液容量、術(shù)后不良反應(yīng),檢測(cè)手術(shù)前后谷氨酸氨基轉(zhuǎn)移酶、天門冬氨酸氨基轉(zhuǎn)移酶、清蛋白、直接膽紅素、總膽紅素、尿素氮、血肌酐水平。結(jié)果兩組手術(shù)前后谷氨酸氨基轉(zhuǎn)移酶、天門冬氨酸氨基轉(zhuǎn)移酶、清蛋白、直接膽紅素、總膽紅素、尿素氮、血肌酐比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。觀察組術(shù)后谷氨酸氨基轉(zhuǎn)移酶、天門冬氨酸氨基轉(zhuǎn)移酶、直接膽紅素、總膽紅素、尿素氮、血肌酐、并發(fā)癥發(fā)生率明顯低于對(duì)照組(P0.05),清蛋白明顯高于對(duì)照組(P0.05)。觀察組術(shù)后排氣時(shí)間、術(shù)后進(jìn)食時(shí)間明顯早于對(duì)照組(P0.05),術(shù)后住院時(shí)間、晶體液量、總輸入量明顯少于對(duì)照組(P0.05),尿量、膠體液量明顯多于對(duì)照組(P0.05)。結(jié)論與傳統(tǒng)補(bǔ)液方案相比,目標(biāo)導(dǎo)向液體治療可減輕腹腔鏡卵巢癌根治術(shù)對(duì)患者肝腎功能的影響,提高安全性。
[Abstract]:Objective to investigate the effect of goal-oriented fluid therapy on liver and kidney function of patients undergoing laparoscopic radical ovarian cancer surgery. Methods 142 patients with laparoscopic radical ovarian cancer were selected from February 2013 to February 2015 in affiliated Hospital of North China University of Technology. The patients were divided into observation group and control group with 71 cases in each group. The control group was given traditional fluid rehydration scheme, the observation group was treated with targeted fluid therapy. The operation time, intraoperative bleeding volume, postoperative exhaust time, postoperative feeding time and postoperative hospitalization time were recorded in the two groups. Body fluid volume, postoperative adverse reactions, detection of glutamate aminotransferase, aspartate aminotransferase, albumin, direct bilirubin, total bilirubin, urea nitrogen before and after operation, Results before and after operation, glutamate aminotransferase, aspartate aminotransferase, albumin, direct bilirubin, total bilirubin, urea nitrogen, The difference of serum creatinine was statistically significant (P 0.05). Glutamate aminotransferase, aspartate aminotransferase, direct bilirubin, total bilirubin, urea nitrogen, serum creatinine were found in the observation group. The incidence of complications was significantly lower than that of the control group (P 0.05), and the level of albumin was significantly higher than that of the control group (P 0.05). The postoperative exhaust time and the postoperative feeding time in the observation group were significantly earlier than those in the control group (P 0.05). The postoperative hospitalization time, the amount of crystal fluid and the total input volume were significantly lower than those in the control group (P 0.05). Conclusion compared with the traditional fluid rehydration regimen, the goal-oriented liquid therapy can reduce the effect of laparoscopic radical operation on liver and kidney function and improve the safety.
