【摘要】：目的:探討自膨式金屬裸支架治療急性肱動脈損傷的安全性及臨床效果。方法:回顧性分析18例急性肱動脈損傷患者資料。所有患者均接受急診血管造影以明確診斷,行血管成形術(shù)及自膨式裸支架植入治療。結(jié)果:18例急性肱動脈損傷的患者的手術(shù)技術(shù)成功率100%,平均手術(shù)時(shí)間為50(30~90)min。13例接受單層自膨式裸支架置入,5例接受雙支架重疊置入術(shù),共置入23枚自膨式裸支架,支架置入后即刻造影顯示損傷肱動脈血流恢復(fù)通暢,假性動脈瘤消失,未見支架狹窄及對比劑外溢。無圍手術(shù)期死亡和嚴(yán)重并發(fā)癥發(fā)生。18例平均隨訪20.2個(gè)月,17例支架內(nèi)血流完全通暢或輕度狹窄(未作處理),1例支架腔內(nèi)狹窄50%,行球囊擴(kuò)張治療后支架內(nèi)血流恢復(fù)通暢;隨訪期間無支架移位、扭曲及斷裂,無缺血壞死,截肢情況。結(jié)論:自膨式金屬裸支架植入治療急性肱動脈損傷是安全可行的,近中期效果滿意。
[Abstract]:Objective: to investigate the safety and clinical effect of self-expandable bare metal stent in the treatment of acute brachial artery injury. Methods: the data of 18 patients with acute brachial artery injury were retrospectively analyzed. Results among 18 patients with acute brachial artery injury, the successful rate of operation was 100, and the average operative time was 50 ~ 3090 min. 13 cases received single layer self-expandable bare stent implantation and 5 cases received double stent overlapping implantation. A total of 23 self-expandable bare stents were implanted. Immediately after implantation, the blood flow of the injured brachial artery recovered and the pseudoaneurysm disappeared. No stent stenosis and contrast media spillover. No perioperative death and severe complications. 18 cases were followed up for an average of 20.2 months. 17 cases had complete flow patency or mild stenosis. After dilatation, the blood flow in the stent recovered unobstructed. There was no stent displacement, distortion and rupture, no ischemic necrosis, amputation during follow-up. Conclusion: Self-expandable bare metal stent implantation is safe and feasible for the treatment of acute brachial artery injury, and the effect is satisfactory in the near and medium term.
