【摘要】：[目的]通過對末節(jié)指骨進(jìn)行解剖、測量分析,探討末節(jié)指骨延長的適宜截骨部位及末節(jié)指骨殘端的外觀重建方法。[方法]2013年10月~2015年1月,收集成人完整前臂標(biāo)本30例,灌注紅色乳膠并低溫保存固定。經(jīng)甲板、甲床、指腹依次分離暴露末節(jié)指骨及其支配血管,并進(jìn)行測量,具體內(nèi)容有:測量末節(jié)指骨全長,甲板長度,甲根長度;關(guān)節(jié)面距伸肌腱止點(diǎn)距離,伸肌腱止點(diǎn)距甲基質(zhì)距離,甲基質(zhì)長度,甲基質(zhì)遠(yuǎn)端距指骨遠(yuǎn)端距離;遠(yuǎn)指間關(guān)節(jié)面距屈肌腱止點(diǎn)距離,關(guān)節(jié)面距滋養(yǎng)動脈入骨點(diǎn)的距離。采用SPSS 18.0軟件包對測量數(shù)據(jù)進(jìn)行統(tǒng)計學(xué)分析。[結(jié)果]拇指:全長為(26.58±1.13)mm,指間關(guān)節(jié)面距伸肌腱止點(diǎn)距離(6.34±1.15)mm,伸肌腱止點(diǎn)距甲基質(zhì)距離(3.25±0.24)mm,甲基質(zhì)長度(5.93±1.41)mm,甲基質(zhì)遠(yuǎn)端距指骨遠(yuǎn)端距離(11.12±0.56)mm,甲根長度為(4.83±0.14)mm,甲板總長度(16.98±0.50)mm,關(guān)節(jié)面距屈肌腱止點(diǎn)距離(12.68±0.61)mm,關(guān)節(jié)面距滋養(yǎng)動脈入骨點(diǎn)的距離(16.51±2.64)mm。手指:全長為(18.63±1.32)mm,關(guān)節(jié)面距伸肌腱止點(diǎn)距離(1.94±0.25)mm,伸肌腱止點(diǎn)距甲基質(zhì)距離(1.77±0.49)mm,甲基質(zhì)長度(5.34±1.19)mm,甲基質(zhì)遠(yuǎn)端距指骨遠(yuǎn)端距離(9.38±0.93)mm,甲根長度為(4.67±0.20)mm,甲板總長度(14.91±0.95)mm,關(guān)節(jié)面距屈肌腱止點(diǎn)距離(7.65±0.81)mm,關(guān)節(jié)面距滋養(yǎng)動脈入骨點(diǎn)的距離(11.24±1.56)mm。[結(jié)論]遠(yuǎn)節(jié)指骨基底是末節(jié)指骨延長的適宜部位。
[Abstract]:[objective] by dissecting and measuring the phalanx of the distal phalanx, to explore the method of reconstruction of the suitable osteotomy site and the stump of the phalanx. [methods] from October 2013 to January 2015, 30 adult cases of intact forearm specimens were collected. The distal phalanx and its innervation vessels were separated and measured through deck, nail bed and finger abdomen. The contents were as follows: the length of phalanx, the length of deck, the length of nail root; The distance from the articular surface to the extensor tendon, the extensor tendon to the nail matrix, the length of the nail matrix, the distance from the distal part of the nail matrix to the distal phalangeal bone, the distance from the distal interphalangeal articular surface to the flexor tendon, The distance from the articular surface to the osseous point of the trophoblastic artery. The measured data were statistically analyzed by SPSS 18.0 software package. [results] the total length of thumb was 26.58 鹵1.13mm, the distance between the interphalangeal articular surface and extensor tendon was 6.34 鹵1.15mm, the distance between extensor tendon and nail matrix was 6.34 鹵1.15mm. The distance between distal nail matrix and distal phalanx was 11.12 鹵0.56 mm, the length of nail root was 4.83 鹵0.14 mm, the total deck length was 16.98 鹵0.50 mm, the distance between articular surface and flexor tendon was 12.68 鹵0.61mm, the distance between articular surface and trophoblastic artery was 16.51 鹵2.64mmm. the total length of finger was 18.63 鹵1.32mm. The distance between extensor tendon and distal phalanx is 1.94 鹵0.25mm, the distance between extensor tendon and nail matrix is 1.77 鹵0.49mm, the length of nail matrix is 5.34 鹵1.19mm, the distance between distal nail matrix and phalanx is 9.38 鹵0.93mm, the length of nail root is 4.67 鹵0.20mm, the total deck length is 14.91 鹵0.95mm, the distance from articular surface to flexor tendon is 7.65 鹵0.81mm, and the distance between articular surface and flexor tendon is 7.65 鹵0.81mm. The distance of the vein into the bone was 11.24 鹵1.56 mm. [conclusion] the base of distal phalangeal bone is the suitable site for the extension of the distal phalangeal bone.
