【摘要】：膝關(guān)節(jié)骨性關(guān)節(jié)炎(knee osteoarthritis,KOA)疼痛非常復(fù)雜,口服藥物不良反應(yīng)多,不利長期應(yīng)用。本文介紹局部外用藥用以控制KOA源性疼痛的現(xiàn)狀。檢索Pub Med、EMBASE、Cochrane臨床對照試驗庫、Cochrane圖書館,檢索時間均從1990年1月至2014年6月。根據(jù)局部外用藥物用以治療KOA的臨床試驗相關(guān)報道,共納入16種藥物,其中歐盟流通非甾體抗炎藥物7種:酮洛芬、雙氯芬酸、氟比洛芬酯、吡羅昔康透皮貼劑、布洛芬、吡羅昔康和尼氟滅酸凝膠;酰胺類局麻藥物1種:利多卡因;阿片類藥物2種:芬太尼和曲馬多;草本藥提取物4種:辣椒素、山金車、橄欖油和紫草科植物。外治法藥物控制KOA疼痛,首推雙氯芬酸、酮洛芬、吡羅昔康,起效快、安全性高。草本類藥物橄欖油、山金車、辣椒素、紫草科植物萃取物可供選擇。局部藥物外治是KOA鎮(zhèn)痛的重要選擇,國外此方面研究方興未艾,國內(nèi)需及時跟進(jìn)。
[Abstract]:Knee osteoarthritis osteoarthritis KOAA is very complicated, oral adverse drug reactions are many, unfavorable long-term application. This paper introduces the current situation of topical administration to control pain caused by KOA. The retrieval time was from January 1990 to June 2014. According to the related reports of clinical trials of topical use of drugs for the treatment of KOA, a total of 16 kinds of drugs were included, including 7 non-steroidal anti-inflammatory drugs in circulation in the European Union: ketoprofen, diclofenac, diclofenac, ketoprofen and diclofenac. Flurbiprofen Ester, Piroxicam Transdermal Patch, Ibuprofen, Piroxicam and Nifluronide Gel; Amide Local Anesthesia 1: lidocaine; opioids 2: fentanyl and Tramadol; herbaceous extract 4 kinds: capsaicin, capsaicin, Herbal medicine for the control of KOA pain, first of all diclofenac, ketoprofen, Piroxicam, quick onset, high safety. Herbaceous drugs olive oil, Yamamoto, capsaicin, Local drug treatment is an important choice for KOA analgesia. The research on this aspect is in the ascendant abroad, and it is necessary to follow up in China.
