【摘要】：背景:后路內(nèi)固定融合系統(tǒng)是治療椎間盤退變導(dǎo)致的慢性背部疼痛的主要方法之一,但固定后不良反應(yīng)較多。作為一種"動態(tài)融合"的替代方法,動態(tài)穩(wěn)定系統(tǒng)可減輕椎間盤/關(guān)節(jié)面的負荷,保持機械負荷下的運動,并限制脊柱節(jié)段的異常運動,理論上可用于修復(fù)椎間盤退變損傷。目的:觀察K-Rod動態(tài)穩(wěn)定系統(tǒng)修復(fù)腰椎退行性病變的有效性及安全性。方法/設(shè)計:研究為前瞻性、單中心、自身對照、臨床試驗。在中國遼寧省,沈陽市骨科醫(yī)院完成。納入腰椎退行性病變患者67例,采用K-Rod動態(tài)穩(wěn)定系統(tǒng)進行修復(fù),隨訪2年。試驗的主要觀察指標(biāo)為固定前、固定后3,6,12,24個月的Oswestry功能障礙指數(shù)變化。試驗的次要觀察指標(biāo)為固定前、固定后3,12,24個月的椎間盤間隙與椎體高度的比值、腰椎前凸角;固定前、固定后3,6,12,24個月的背部疼痛目測類比評分及腰椎X射線形態(tài);固定后3,6,12,24個月不良反應(yīng)發(fā)生率。試驗已在北美臨床試驗注冊中心注冊(NCT03214042)。試驗經(jīng)中國遼寧省,沈陽市骨科醫(yī)院倫理委員會批準(zhǔn)。研究符合世界醫(yī)學(xué)會制定的《赫爾辛基宣言》的要求。參與者本人對治療方案和過程均知情同意,并簽署知情同意書。討論:試驗旨在驗證K-Rod動態(tài)穩(wěn)定系統(tǒng)修復(fù)腰椎退行性損傷安全有效,為臨床腰椎退行性病變的治療提供參考依據(jù)。目前部分試驗結(jié)果顯示,患者固定后24個月的Oswestry功能障礙指數(shù)及背部疼痛目測類比評分均較固定前相比明顯改善(P0.01),但患者術(shù)前與固定后各時間點椎間盤間隙與椎體高度的比值及腰椎前凸角差異無顯著性意義(P0.05)。結(jié)果說明,對腰椎退行性病變患者而言,K-Rod動態(tài)穩(wěn)定系統(tǒng)固定修復(fù)后可減輕疼痛并改善腰部功能。
[Abstract]:Background: posterior internal fixation fusion system is one of the main methods for the treatment of chronic back pain caused by disc degeneration, but there are more adverse reactions after fixation. The dynamic stabilization system can reduce the load of intervertebral disc / articular surface, maintain movement under mechanical load, and restrict abnormal movement of spinal segment. Objective: to observe the efficacy and safety of K-Rod dynamic stabilization system in the treatment of lumbar degenerative lesions. Clinical trial. 67 patients with lumbar degenerative diseases were treated with K-Rod dynamic stabilization system in Shenyang Orthopedic Hospital, Liaoning Province, China, and followed up for 2 years. The changes of Oswestry dysfunction index were observed at 12 and 24 months after fixation. The main indexes of the experiment were the ratio of intervertebral disc space to vertebral body height before fixation, 3o 12,24 months after fixation, lumbar kyphosis angle, before fixation, and before fixation. Visual analogue score of back pain and X-ray morphology of lumbar vertebrae, incidence of adverse reactions in 36 cases and 24 months after fixation. NCT03214042 was registered at the North American Clinical trial Registry. The trial was conducted in Liaoning Province, China. Approved by the Ethics Committee of Shenyang Orthopaedics Hospital. The study meets the requirements of the Helsinki Declaration formulated by the World Medical Association. The purpose of the study was to verify the safety and effectiveness of K-Rod dynamic stabilization system in the repair of lumbar degenerative injury, and to provide a reference for the treatment of clinical lumbar degenerative lesions. The Oswestry dysfunction index and visual analogue score of back pain in patients 24 months after fixation were significantly improved compared with those before fixation, but the ratio of intervertebral disc space to vertebral body height and lumbar kyphosis at each time point before and after fixation were significantly improved. There was no significant difference in angle (P 0.05). For patients with lumbar degenerative disease, K-Rod dynamic stabilization system fixation can relieve pain and improve lumbar function.
