軟骨下骨質(zhì)疏松對(duì)骨關(guān)節(jié)炎病程發(fā)展的促進(jìn)作用

【摘要】:目的:探討軟骨下骨質(zhì)疏松對(duì)骨關(guān)節(jié)炎病程發(fā)展的促進(jìn)作用。方法:選取3月齡雌性Wistar大鼠120只,所有大鼠均健康未孕,均接受2周的適應(yīng)性飼養(yǎng)。依據(jù)體重將這些大鼠分為去勢(shì)組(n=30)、去勢(shì)對(duì)照組(n=30)、維甲酸組(n=30)、維甲酸對(duì)照組(n=30)四組,按組分籠飼養(yǎng)。制備大鼠模型,采集標(biāo)本并制備關(guān)節(jié)標(biāo)本病理切片,進(jìn)行病理切片染色,然后對(duì)四組大鼠的左膝關(guān)節(jié)BMD、脛骨關(guān)節(jié)軟骨厚度、病理學(xué)肉眼觀察結(jié)果評(píng)分進(jìn)行統(tǒng)計(jì)分析。結(jié)果:8周、12周時(shí)維甲酸組大鼠的左膝關(guān)節(jié)BMD均顯著低于去勢(shì)組(P0.05),脛骨關(guān)節(jié)軟骨厚度均顯著低于去勢(shì)組(P0.05);8周時(shí)維甲酸組、去勢(shì)組大鼠的脛骨關(guān)節(jié)軟骨病理學(xué)肉眼觀察結(jié)果評(píng)分0分、1分、2分、3分、4分比例之間的差異均不顯著(P0.05),12周時(shí)維甲酸組大鼠的4分比例顯著高于去勢(shì)組(P0.05),但0分、1分、2分、3分比例之間的差異均不顯著(P0.05)。結(jié)論:軟骨下骨質(zhì)疏松對(duì)骨關(guān)節(jié)炎病程發(fā)展有促進(jìn)作用,隨著軟骨下骨質(zhì)疏松的加重,骨關(guān)節(jié)炎不斷加重。臨床在對(duì)骨性關(guān)節(jié)炎進(jìn)行治療的過(guò)程中應(yīng)該同時(shí)對(duì)骨質(zhì)疏松進(jìn)行治療,只有這樣才能收到較好的治療效果。
[Abstract]:Objective: to investigate the effect of subchondral osteoporosis on the development of osteoarthritis. Methods: 120 3-month-old female Wistar rats were selected. All the rats were fed adaptively for 2 weeks. The rats were divided into four groups according to their body weight: castration group, castrated control group, retinoic acid group, retinoic acid control group and retinoic acid control group. The specimens were collected and the pathological sections of joint specimens were prepared and stained with pathological sections. Then the left knee joint BMD and tibial articular cartilage thickness of the four groups of rats were studied. Results the BMD of left knee in retinoic acid group was significantly lower than that of castrated group (P 0.05), and the thickness of tibial articular cartilage was significantly lower than that of ovariectomized group at 8 weeks. The pathological observation of tibial articular cartilage in ovariectomized rats showed no significant difference between 0, 1, 2, 3 and 4 points. The ratio of 4 points in the retinoic acid group was significantly higher than that in the castrated group at 12 weeks. Conclusion: subchondral osteoporosis can promote the development of osteoarthritis. With the exacerbation of subchondral osteoporosis, osteoarthritis is becoming more and more serious. In the course of treatment of osteoarthritis, osteoporosis should be treated at the same time, only in this way can better therapeutic effect be obtained.
