【摘要】：目的通過(guò)構(gòu)建上頸椎有限元模型研究?jī)煞N齒狀突切除術(shù)對(duì)顱頸交界區(qū)(craniocervical junction,CCJ)穩(wěn)定性影響的生物力學(xué)差異。方法采用有限元軟件構(gòu)建Oc-C2的正常上頸椎實(shí)體模型,然后在正常模型的基礎(chǔ)上構(gòu)建常規(guī)齒狀突切除模型及保留C1前弓的微創(chuàng)齒狀突切除模型,進(jìn)而在有限元軟件ABAQUS v6.9.1.中分析各種模型的生物力學(xué)差異,從而得出齒狀突切除術(shù)中保留C1前弓的生物力學(xué)優(yōu)勢(shì)。結(jié)果常規(guī)齒狀突切除術(shù)對(duì)C1-C2的活動(dòng)度影響較Oc-C1顯著,各向活動(dòng)度增加分別為前屈23%、后伸97%、側(cè)彎231%、軸向旋轉(zhuǎn)349%;對(duì)Oc-C1的穩(wěn)定性同樣有明顯影響,其中Oc-C1后伸活動(dòng)度影響最大,增加達(dá)196%。保留C1前弓的齒狀突切除術(shù)可明顯縮減C1-C2軸向旋轉(zhuǎn)活動(dòng)度的增加,實(shí)驗(yàn)中僅出現(xiàn)輕度代償性的活動(dòng)度減少;而其他方向活動(dòng)度仍有異常的增加(Oc-C1后伸活動(dòng)度增加167%、C1-C2側(cè)彎活動(dòng)度增加181%)。結(jié)論齒狀突切除術(shù)對(duì)C1-C2的活動(dòng)度影響顯著,但對(duì)Oc-C1的活動(dòng)度同樣有明顯影響。保留C1前弓的齒狀突切除術(shù)可保護(hù)脊柱軸向旋轉(zhuǎn)的活動(dòng)度,但仍存在脊柱不穩(wěn)定因素。
[Abstract]:Objective to construct the finite element model of the upper cervical spine of two odontoid resection of craniocervical junction (craniocervical junction, CCJ). Cervical biomechanical differences affect the stability of solid model construction method Oc-C2 using finite element software to normal, and then construct the odontoid process of conventional minimally invasive resection of odontoid process model and retention of C1 anterior arch resection model on the basis of the normal model, and biomechanical analysis of differences between the various models in finite element software ABAQUS v6.9.1., so that odontoidectomy retains the biomechanical advantages of anterior arch of C1 during operation. The results of conventional odontoid resection of effect of surgery on the C1-C2 than the Oc-C1 activity increased significantly, anisotropy respectively reach 23%, extension 97%, 231% and 349% lateral bending, axial rotation; stability of Oc-C1 have the same effect, the Oc-C1 extension activity increased to the greatest impact, 196%. keep C1 anterior arch The odontoid resection can significantly reduce the increase of C1-C2 axial rotation degree, only mild compensatory activity decreased in the other direction and experiment; activity is abnormal increase (Oc-C1 extension activity increased 167% C1-C2, lateral bending activity increased 181%). Conclusion odontoid resection surgery on the effect of activity C1-C2 is significantly, but the Oc-C1 activity also had obvious effects. Keep C1 anterior arch of odontoid resection can protect the activity of spinal rotation axis, but there are still some spinal instability.
