【摘要】：探討3種不同的吸痰負(fù)壓對(duì)重型顱腦損傷患者的影響,以尋找重型顱腦損傷患者最佳的吸痰負(fù)壓。選取某三甲醫(yī)院的ICU重癥顱腦損傷患者46例,當(dāng)患者出現(xiàn)吸痰指征時(shí),分別以3種不同吸痰負(fù)壓(-75 mmHg,-150 mmHg,-300 mmHg)對(duì)患者行密閉式吸痰,在吸痰前2 min、吸痰后5 min以及吸痰后10 min時(shí)記錄患者的氧動(dòng)力學(xué)和血流動(dòng)力學(xué)變化,同時(shí)記錄患者的吸痰時(shí)間間隔和效果。吸痰負(fù)壓為-150 mmHg和-300 mmHg的吸痰效果和吸痰間隔時(shí)間要優(yōu)于-75 mmHg的吸痰負(fù)壓(p0.05),且吸痰負(fù)壓為-150 mmHg時(shí),吸痰的間隔時(shí)間最長(zhǎng)(78.3±29.8)min;不同吸痰負(fù)壓組不同時(shí)間點(diǎn)的氧分壓,二氧化碳分壓、收縮壓、舒張壓和平均動(dòng)脈壓差異有統(tǒng)計(jì)學(xué)意義(p0.01),且當(dāng)吸痰負(fù)壓是-150 mmHg時(shí)、吸痰后5 min和10 min時(shí)氧分壓相對(duì)于吸痰前有明顯的改善(p=0.00),當(dāng)吸痰負(fù)壓為-300 mmHg時(shí),吸痰后5 min相較于吸痰前收縮壓、舒張壓和平均動(dòng)脈壓都明顯提高,具有統(tǒng)計(jì)學(xué)意義(p=0.00)。-150 mmHg的吸痰負(fù)壓是安全和有效的適合重型顱腦損傷患者的吸痰負(fù)壓。
[Abstract]:To explore the effect of three different sputum suction negative pressure on patients with severe craniocerebral injury in order to find the best suction negative pressure in severe craniocerebral injury 46 patients with ICU severe craniocerebral injury were selected. When the indication of sputum aspiration appeared, the patients were inhaled with three different negative pressure of sputum suction (-75mmHgGX -150mmHgGN -300mmHg). The changes of oxygen dynamics and hemodynamics were recorded 2 minutes before sputum aspiration, 5 min after sputum aspiration and 10 min after sputum aspiration. At the same time, the time interval and effect of sputum aspiration were recorded. The suction effect and interval time of suction negative pressure of -150 mmHg and -300 mmHg were better than that of -75 mmHg of sucking sputum negative pressure. P0.05). When the negative pressure was -150 mmHg, the interval of sputum aspiration was 78.3 鹵29.8 minutes. There were significant differences in oxygen partial pressure, carbon dioxide partial pressure, systolic pressure, diastolic pressure and mean arterial pressure between different sputum suction negative pressure groups at different time points (p 0.01). When the sputum suction pressure was -150 mmHg, the oxygen partial pressure at 5 min and 10 min after sputum aspiration was significantly improved compared with that before sputum aspiration. When the negative sputum pressure was -300 mmHg, the systolic blood pressure, diastolic pressure and mean arterial pressure were significantly increased 5 min after sputum aspiration compared with those before sputum aspiration. The sputum suction negative pressure of p0. 00m. -150 mmHg is safe and effective for severe craniocerebral injury patients.
