【摘要】：背景:在全髖關(guān)節(jié)置換術(shù)中,靜脈與局部應(yīng)用氨甲環(huán)酸可顯著減少圍手術(shù)期的失血量和輸血率,但目前在臨床工作中對氨甲環(huán)酸的應(yīng)用方式仍有爭議。目的:應(yīng)用Cochrane系統(tǒng)方法評價(jià)氨甲環(huán)酸靜脈聯(lián)合局部用藥與單劑量靜脈用藥在全髖關(guān)節(jié)置換術(shù)中的應(yīng)用效果及安全性。方法:計(jì)算機(jī)檢索Cochrane Library,Pub Med,Ovid,EMBASE,CBM,萬方,VIP,CNKI等數(shù)據(jù)庫,檢索時(shí)間從建庫至2016年7月,檢索有關(guān)全髖關(guān)節(jié)置換術(shù)中氨甲環(huán)酸靜脈聯(lián)合局部與單劑量靜脈應(yīng)用的隨機(jī)對照試驗(yàn)文獻(xiàn),對2種用藥方式的總失血量、術(shù)中失血量、術(shù)后失血量、輸血率、血栓形成、手術(shù)時(shí)間、住院時(shí)間進(jìn)行Meta分析。結(jié)果與結(jié)論:(1)共納入7篇隨機(jī)對照試驗(yàn),包含620例患者;(2)Meta分析結(jié)果顯示,靜脈聯(lián)合局部組在總失血量、術(shù)中失血量、術(shù)后出血量少、輸血率方面較單劑量靜脈用藥組有明顯優(yōu)勢(P0.05),在血栓形成、手術(shù)時(shí)間、住院時(shí)間2組差異無顯著性意義(P0.05);(3)結(jié)果表明,與單劑量靜脈用藥相比,在全髖關(guān)節(jié)置換術(shù)中氨甲環(huán)酸靜脈聯(lián)合局部應(yīng)用可明顯減少失血量及輸血率,但不增加血栓形成率。
[Abstract]:Background: in total hip arthroplasty, intravenous and local application of carbamoic acid can significantly reduce perioperative blood loss and blood transfusion rate. However, there is still controversy about the application of methachloroic acid in clinical work at present. Cochrane system was used to evaluate the efficacy and safety of combined local and single dose intravenous therapy in total hip arthroplasty. The computer retrieves the Cochrane Library. Pub Med#en0# Ovid Ovid EMBASEN CBM, Wanfang VIPN CNKI and other databases, the retrieval time is from the construction of the database to July 2016. To search the literature of randomized controlled trials on the combination of local and single dose veins in total hip arthroplasty, the total blood loss, intraoperative blood loss, postoperative blood loss and blood transfusion rate of the two drugs were studied. Thrombus formation, operation time, and hospitalization time were analyzed by Meta. Results and conclusion: a total of 7 randomized controlled trials, including 620 patients, were included in the study. The results of Meta-analysis showed that the total blood loss, intraoperative blood loss and postoperative blood loss were less in the combined intravenous group, and the blood transfusion rate was significantly higher than that in the single dose intravenous drug group (P0.05). There was no significant difference in thrombus formation, operation time and hospitalization time between the two groups (P 0.05). The results showed that compared with single dose intravenous injection, combined local application of carbamoic acid vein in total hip arthroplasty could significantly reduce blood loss and blood transfusion rate, but did not increase the rate of thrombosis.
