【摘要】：目的探討右美托咪定(Dex)聯(lián)合降壓藥用于DebakeyⅠ型主動脈夾層患者對術(shù)前血壓、心率的臨床效果。方法收集2013年1月至2016年2月大連醫(yī)科大學(xué)附屬第一醫(yī)院心臟重癥監(jiān)護(hù)室所有DebakeyⅠ型主動脈夾層合并高血壓患者的臨床相關(guān)資料,根據(jù)Dex的使用情況將患者隨機(jī)分為非Dex組(Non-Dex組,常規(guī)降壓藥物)和Dex組(常規(guī)藥物+靜脈泵注Dex),各26例。觀察患者降壓藥物治療期間心率、血壓等臨床指標(biāo),比較兩組患者藥物使用情況及療效。結(jié)果與Non-Dex組比較,Dex組在不同時間段使用硝普鈉、烏拉地爾、硝苯地平、艾司洛爾、美托洛爾、替米沙坦等降壓藥物劑量明顯減少(P0.05);平均動脈壓(MAP)及心率降低(均P0.05);6h內(nèi)Dex組MAP、心率達(dá)標(biāo)率分別為76.92%、80.77%,明顯高于Non-Dex組(均P0.05)。結(jié)論 Dex聯(lián)合降壓藥用于DebakeyⅠ型主動脈夾層患者,可明顯減少服用降壓藥劑量,有效控制血壓、心率,縮短目標(biāo)血壓、心率達(dá)標(biāo)時間,提高達(dá)標(biāo)率。
[Abstract]:Objective to investigate the preoperative blood pressure of patients with Debakey type I aortic dissection treated with dexmetomidine combined with antihypertensive drugs. Methods the clinical effects of heart rate were collected from January 2013 to February 2016 in all patients with Debakey type I aortic dissection complicated with hypertension in Cardiac intensive Care Unit of the first affiliated Hospital of Dalian Medical University. Clinical data. According to the use of Dex, the patients were randomly divided into non-Dex group (non-Dex group, conventional antihypertensive drug) group and Dex group (Dexx group). The clinical indexes such as heart rate and blood pressure were observed and compared between the two groups. The results were compared with those in the Non-Dex group. The dosage of sodium nitroprusside, urapidil, nifedipine, esmolol, metoprolol and telmisartan were significantly decreased in Dex group. The mean arterial pressure (MAPP) and heart rate (P 0.05) were decreased. Within 6 hours, the rate of Dex and heart rate were 76.92% and 80.77%, respectively. Conclusion Dex combined with antihypertensive drugs can significantly reduce the dosage of antihypertensive drugs in patients with Debakey type 鈪，
