【摘要】：目的:采用有限元分析方法研究單髁置換術(shù)后膝關(guān)節(jié)表面應(yīng)力變化,為深入了解單髁置換術(shù)優(yōu)勢(shì)及劣勢(shì)提供依據(jù)。方法:對(duì)6例單髁置換術(shù)后及6例正常人膝關(guān)節(jié)屈伸活動(dòng)時(shí)關(guān)節(jié)表面應(yīng)力變化進(jìn)行動(dòng)態(tài)有限元分析,并與項(xiàng)目組前期全膝關(guān)節(jié)置換術(shù)后關(guān)節(jié)表面應(yīng)力分析進(jìn)行對(duì)比。結(jié)果:正常組屈曲30°以后內(nèi)側(cè)間室應(yīng)力大于外側(cè)間室(P0.05);單髁置換組屈曲0°、60°、90°時(shí)內(nèi)側(cè)間室應(yīng)力大于外側(cè)間室(P0.05);全膝置換組在所有屈曲角度下內(nèi)側(cè)間室應(yīng)力大于外側(cè)間室(P0.05)。屈曲60°及90°時(shí)單髁置換組內(nèi)側(cè)間室應(yīng)力大于全膝置換組(P0.05);所有屈曲角度下,單髁置換組外側(cè)間室應(yīng)力大于全膝置換組外側(cè)間室應(yīng)力(P0.05);屈曲0°、30°、60°及120°時(shí),單髁置換組外側(cè)間室應(yīng)力都大于正常膝組(P0.05)。結(jié)論:單髁置換術(shù)可增加膝關(guān)節(jié)關(guān)節(jié)面應(yīng)力及加速對(duì)側(cè)間室關(guān)節(jié)炎發(fā)展,從而影響手術(shù)遠(yuǎn)期療效,但在嚴(yán)格把握手術(shù)適應(yīng)癥、優(yōu)化手術(shù)操作、假體適當(dāng)改良、指導(dǎo)患者合理康復(fù)后,該術(shù)式仍具有優(yōu)勢(shì)。
[Abstract]:Objective: to study the surface stress changes of knee joint after single condylar replacement by finite element analysis (FEM). Methods: the dynamic finite element analysis was carried out on the surface stress of 6 cases of single condylar replacement and 6 cases of normal knee joint during flexion and extension activity in order to provide the basis for further understanding the advantages and disadvantages of single condylar replacement. Results: the stress of medial interventricular after 30 擄flexion in normal group was greater than that of lateral interventricular chamber (P0.05). In the single condylar replacement group, the stress of the medial interventricular was greater than that of the lateral interventricular compartment at the flexion of 0 擄~ 60 擄~ 90 擄; The stress of medial interventricular in total knee replacement group was greater than that in lateral interventricular chamber at all flexion angles, and the stress of medial interventricular chamber in single condylar replacement group was greater than that in whole knee replacement group at flexion of 60 擄and 90 擄. At all flexion angles, the stress of the lateral interventricular in the single condylar replacement group was higher than that in the whole knee replacement group (P 0.05). When flexion was 0 擄, 30 擄, 60 擄and 120 擄. Conclusion: single condyle replacement can increase the stress of articular surface of knee joint and accelerate the development of contralateral ventricular arthritis, thus affecting the long-term outcome of the operation. But after strictly grasping the indication of operation, optimizing the operation operation, improving the prosthesis properly and guiding the patients to recover reasonably, the operation method still has advantages.
