【摘要】：研究目的:評(píng)估高齡患者髖部骨折分型及其在人工股骨頭置換術(shù)的假體選擇的指導(dǎo)作用。針對(duì)高齡患者常見(jiàn)的股骨頸及轉(zhuǎn)子間的骨折,我們把它們統(tǒng)稱(chēng)為髖部骨折,擬定一個(gè)基于股骨距破壞程度的高齡患者髖部骨折分型。本文旨在根據(jù)我組在為不同類(lèi)型髖部骨折行人工股骨頭置換術(shù)時(shí)選用不同類(lèi)型假體的治療及隨訪(fǎng)結(jié)果,為今后的高齡患者髖部骨折行人工股骨頭置換術(shù)的假體選擇提供參考。研究方法:收集并分析我組于2012年1月～2016年10月期間共收治≥80歲的高齡患者因髖部骨折行人工股骨頭置換術(shù)55例,患者年齡為80-97歲,其中男22例,女33例,平均年齡87歲,根據(jù)患者術(shù)前影像學(xué)資料顯示骨折對(duì)股骨距破壞程度,把所有患者分為:A型、B1型、B2型、C型,并選用相應(yīng)的股骨假體。所有患者均行人工股骨頭置換術(shù)治療,應(yīng)用復(fù)查X光片觀(guān)察骨折愈合情況并結(jié)合髖關(guān)節(jié)Harris評(píng)分進(jìn)行隨訪(fǎng),定期評(píng)估所有患者術(shù)后病情恢復(fù)情況及臨床療效,最后分別統(tǒng)計(jì)比較A型、B型(B1和B2)、C型骨折組患者手術(shù)前后Harris評(píng)分差異是否相同。結(jié)果:所有手術(shù)治療的55例患者均獲得隨訪(fǎng),隨訪(fǎng)時(shí)間4～52個(gè)月(平均18個(gè)月),術(shù)后X片提示假體位置及角度良好,患肢較對(duì)側(cè)基本等長(zhǎng),其中2例較對(duì)側(cè)短縮1cm�？傮wHarris評(píng)分65～91分,平均為79.3分,優(yōu)良率達(dá)到100%;3組患者術(shù)前術(shù)后Harris評(píng)分差值存在差異,差異有統(tǒng)計(jì)學(xué)意義(P0.01),且評(píng)分差值C型B型A型。結(jié)論:對(duì)于此類(lèi)高齡髖部骨折患者,成功的人工股骨頭置換術(shù)除了需要手術(shù)醫(yī)生精湛的操作技術(shù),正確選擇合適的假體是其關(guān)鍵。因此,我們通過(guò)制定一個(gè)髖部骨折新的綜合分型,只描述髖部骨折的核心關(guān)注點(diǎn)——股骨距完整性,給手術(shù)醫(yī)生選擇假體提供更直觀(guān)的建議。從本次研究的患者術(shù)后恢復(fù)可見(jiàn),此分型也是具有相當(dāng)?shù)目尚行浴?br/>[Abstract]:Objective: to evaluate the classification of hip fracture in elderly patients and its guiding role in prosthetic selection of artificial femoral head replacement. We call them hip fractures. The purpose of this study was to select different types of prostheses for the treatment and follow-up of different types of femoral head replacement for different types of hip fractures in our group according to the classification of hip fractures based on the degree of fracture of the femur. To provide a reference for the selection of prosthesis for hip fracture replacement in elderly patients in the future. From January 2012 to October 2016, we collected and analyzed 55 cases of artificial femoral head replacement for hip fracture in our group. The age of the patients was 80-97 years old, including 22 males and 33 females with an average age of 87 years. Type B1, type B2, type C, and the corresponding femoral prosthesis were selected. All the patients were treated with artificial femoral head replacement. X-ray examination was used to observe fracture healing and Harris score of hip joint was used to follow up and evaluate the recovery and clinical effect of all patients regularly. Finally, A type was compared statistically. The difference of Harris score before and after operation in patients with type B and type B 2 fracture was the same or not. Results: 55 patients with all kinds of surgical treatment were followed up. The follow-up time was 4 ~ 52 months (mean 18 months). X ray showed that the position and angle of the prosthesis were good and the affected limb was equal to the opposite side. The total Harris score was 651.The average score was 79.3, and the excellent and good rate was 100; There was significant difference in Harris scores between the three groups before and after operation, and the difference was significant (P 0.01), and the score difference was C type B A. Conclusion: for this kind of elderly patients with hip fracture. Successful artificial femoral head replacement requires the masterly operation of the surgeon and the proper selection of the prosthesis is the key. Therefore, we develop a new comprehensive classification of hip fractures. We only describe the core concern of hip fracture, the integrity of the femoral talus, and provide more direct advice to the surgeon on the choice of prosthesis. From the postoperative recovery of the patients in this study, this classification is also quite feasible.
