【摘要】：目的探討應(yīng)用內(nèi)踝上皮支皮瓣修復(fù)糖尿病足部潰瘍的臨床療效。方法 2012年3月至2015年3月,在內(nèi)科治療控制血糖的基礎(chǔ)上,對糖尿病足合并Wagner分級2級以上的足部潰瘍采用內(nèi)踝上皮支皮瓣修復(fù)32例共38足。結(jié)果術(shù)后皮瓣血運(yùn)良好,2例皮瓣遠(yuǎn)端小面積壞死,經(jīng)換藥后治愈,l例半年后因糖尿病血糖控制不良,外傷后傷口不愈合再次發(fā)生潰瘍,并出現(xiàn)糖尿病足壞疽,行截肢處理。本組病例隨訪2~18個月,皮瓣成活良好,質(zhì)地優(yōu)良,耐磨壓,大部分恢復(fù)保護(hù)性感覺,患足可負(fù)重行走,功能恢復(fù)滿意。結(jié)論應(yīng)用內(nèi)踝上皮支皮瓣修復(fù)糖尿病足部潰瘍臨床效果良好,手術(shù)操作簡便易行,能有效的提高患者生活質(zhì)量。
[Abstract]:Objective to investigate the clinical effect of treating diabetic foot ulcer with medial malleolus epithelial branch flap. Methods from March 2012 to March 2015, blood glucose was controlled by internal medicine. 38 feet were repaired with medial malleolus epithelium branch flap for foot ulcer with Wagner grade 2 or more. Results there were 2 cases of small area necrosis of the distal end of the flap. After dressing change, 1 case was cured half a year later because of poor control of diabetic blood sugar, re-ulcer of wound nonunion after trauma, and gangrene of diabetic foot, which were treated with amputation. The patients were followed up for 2 ~ 18 months. The skin flap survived well, had good texture, wear-resistant pressure, most of the protective feeling was restored, the affected foot could walk with load, and the function was satisfactory. Conclusion the application of medial malleolus epithelial branch flap in the repair of diabetic foot ulcers is effective. The operation is simple and easy, and can effectively improve the quality of life of patients.
