【摘要】:目的用普通膝關(guān)節(jié)假體行TKA后,膝關(guān)節(jié)安全活動(dòng)范圍約為100°~110°,如要完成包括下跪、下蹲等高度屈曲動(dòng)作,必須選用高屈曲度設(shè)計(jì)假體。本文研究觀察為膝關(guān)節(jié)高度屈曲而設(shè)計(jì)的Nex Gen LPS高屈曲型人工全膝關(guān)節(jié)假體置換術(shù)后中遠(yuǎn)期效果。方法回顧性分析我院106例(120膝)接受Nex Gen LPS高屈曲型人工全膝關(guān)節(jié)假體置換術(shù)后8~11年的患者的相關(guān)數(shù)據(jù),包括術(shù)前、術(shù)后測(cè)量的膝關(guān)節(jié)屈曲度,及根據(jù)KSS膝關(guān)節(jié)評(píng)分系統(tǒng)進(jìn)行評(píng)分。結(jié)果獲隨訪106例(120膝),平均隨訪時(shí)間(123.5±3.2)個(gè)月(96~134個(gè)月),KSS術(shù)前膝評(píng)分平均為(46.3±15.6)分、功能評(píng)分平均為(49.5±18.6)分,術(shù)后末次隨訪膝評(píng)分平均為(92.7±6.1)分、功能評(píng)分平均為(81.6±17.8)分,術(shù)前膝屈曲度為90°、90°~100°、100°的三組患者術(shù)后屈曲度平均增加(37.6°±7.2°),(25.8°±1.9°)和(6.9°±2.1°),1例出現(xiàn)假體無(wú)菌性松動(dòng),1例發(fā)生感染。結(jié)論Nex Gen LPS高屈曲型人工膝關(guān)節(jié)假體通過(guò)設(shè)計(jì)上的改進(jìn)使理論上能達(dá)到高度屈曲,但膝關(guān)節(jié)假體置換后的屈曲度受術(shù)前屈曲度等多因素影響,術(shù)前活動(dòng)度較差的患者術(shù)后獲益更大,并且假體松動(dòng)、感染等并發(fā)癥發(fā)生率較低,術(shù)后總體中遠(yuǎn)期臨床效果滿意。
[Abstract]:Objective after the knee joint prosthesis was performed with TKA, the safe range of knee joint movement was about 100 擄to 110 擄, including kneeling, squat and other high flexion movements. We must choose the high buckling degree to design the prosthesis. In this paper, we observed the Nex Gen designed for high flexion of the knee joint. LPS high flexion total knee prosthesis replacement was performed in the middle and long term. #number0# knees) received Nex Gen LPS high-flexion total knee prosthesis replacement 8 ~ 11 years after the relevant data. The flexion of knee joint was measured before and after operation, and evaluated according to KSS knee scoring system. The average follow-up time was 123.5 鹵3.2 months, 96 ~ 134 months and the average preoperative knee score was 46.3 鹵15.6). The average score of functional score was 49.5 鹵18.6, the average score of the last follow-up was 92.7 鹵6.1, and the average score of functional score was 81.6 鹵17.8. The average postoperative flexion of knee flexion was 37.6 擄鹵7.2 擄in the three groups with preoperative flexion of 90 擄to 90 擄and 100 擄to 100 擄. There was aseptic loosening of prosthesis in 1 case (25.8 擄鹵1.9 擄) and 6.9 擄鹵2.1 擄(P < 0.05). Conclusion Nex Gen LPS high flexion artificial knee prosthesis can achieve high flexion theoretically by improving the design of knee prosthesis. However, the degree of flexion after knee prosthesis replacement was affected by multiple factors such as preoperative flexion. The patients with poor preoperative movement had more benefit after operation, and the complications such as loosening of prosthesis and infection were lower. The overall mid-and long-term clinical results were satisfactory.
