【摘要】：目的評(píng)價(jià)全膝關(guān)節(jié)置換(TKA)術(shù)中注射雞尾酒式鎮(zhèn)痛混合劑對(duì)膝關(guān)節(jié)中期功能和屈伸活動(dòng)度的影響。方法筆者自2006-05—2007-05診治80例行單側(cè)全膝關(guān)節(jié)置換的膝關(guān)節(jié)骨性關(guān)節(jié)炎,隨機(jī)分為試驗(yàn)組和對(duì)照組,對(duì)比2組術(shù)后中期隨訪時(shí)美國(guó)膝關(guān)節(jié)協(xié)會(huì)(KSS)評(píng)分和膝關(guān)節(jié)活動(dòng)度(ROM)的差異。結(jié)果 64例獲隨訪45~69個(gè)月,平均61個(gè)月,試驗(yàn)組有效隨訪31例,對(duì)照組有效隨訪33例。試驗(yàn)組KSS評(píng)分(91.9±4.4)分,屈膝角度(129.2±6.9)°,伸膝角度(0.5±1.2)°;對(duì)照組KSS評(píng)分(87.5±4.9)分,屈膝角度(121.5±7.1)°,伸膝角度(0.3±1.1)°,試驗(yàn)組KSS評(píng)分及主動(dòng)屈膝活動(dòng)度指標(biāo)優(yōu)于對(duì)照組,2組KSS評(píng)分(t=3.77,P=0.001)、主動(dòng)屈膝活動(dòng)度(t=4.40,P=0.001)比較差異有統(tǒng)計(jì)學(xué)意義,主動(dòng)伸膝活動(dòng)度2組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(t=0.70,P=0.492)。結(jié)論雞尾酒式鎮(zhèn)痛混合劑改善TKA術(shù)后膝關(guān)節(jié)中期功能效果顯著。
[Abstract]:Objective to evaluate the total knee arthroplasty (TKA) effects of intraoperative injection of cocktail analgesic mixture on extension activity of knee joint flexion and mid function. Methods 80 patients with knee osteoarthritis who underwent unilateral total knee arthroplasty from 2006-05 to 2007-05 were randomly divided into the experimental group and the control group. The difference between the 2 knee joint association (KSS) score and the knee range of motion (ROM) was compared between the two groups at the mid term follow-up. Results 64 cases were followed up for 45~69 months, with an average of 61 months. 31 cases were followed up effectively in the experimental group and 33 cases were followed up effectively in the control group. The KSS scores of test group (91.9 + 4.4), flexion angle (129.2 + 6.9) degrees of knee extension angle (0.5 + 1.2); control group KSS score (87.5 + 4.9), flexion angle (121.5 + 7.1) degrees of knee extension angle (0.3 + 1.1) degree, KSS score in the treatment group active flexion and activity index than the control group, the KSS scores of the 2 groups (t=3.77, P=0.001), active flexion activity (t=4.40, P=0.001) had a significant difference, active knee extensor activity of the 2 groups had no statistically significant difference (t=0.70, P=0.492). Conclusion cocktail analgesic mixture improves TKA postoperative knee function middle effect.
