二維斑點(diǎn)追蹤技術(shù)評(píng)價(jià)冠脈搭橋患者術(shù)后左心室功能的變化

【摘要】:目的應(yīng)用二維斑點(diǎn)追蹤技術(shù)評(píng)價(jià)冠脈搭橋(CABG)患者術(shù)后左心室功能的變化。方法對(duì)30例CABG患者,分別于術(shù)前、術(shù)后1、3、6個(gè)月行超聲心動(dòng)圖檢查,測量左室二腔心、三腔心、四腔心切面長軸應(yīng)變及應(yīng)變率,獲得左室長軸總體收縮期應(yīng)變(GLSs)及應(yīng)變率(GLSRs)、舒張?jiān)缙趹?yīng)變率(GSRe)、等容舒張期應(yīng)變率(GSRivr),比較常規(guī)超聲心動(dòng)圖參數(shù)及二維應(yīng)變參數(shù)在冠脈搭橋術(shù)后的變化。結(jié)果(1)術(shù)后1個(gè)月左室射血分?jǐn)?shù)(EF)較術(shù)前減低且有統(tǒng)計(jì)學(xué)意義(P0.05),術(shù)后3、6個(gè)月的EF值升高,但較術(shù)前無明顯差異。術(shù)后1、3、6個(gè)月E、A、E/A均較術(shù)前無明顯差異。(2)術(shù)后1個(gè)月GLSs從-17.35%±0.98%降低至-15.39%±0.95%,GLSRs從-0.85±0.09降低至-0.74±0.14,P值均0.05,術(shù)后3個(gè)月逐漸恢復(fù)至術(shù)前水平,但無統(tǒng)計(jì)學(xué)差異,術(shù)后6個(gè)月GLSs-18.71%±0.58%和GLSRs(-0.94±0.15)均較術(shù)前增加,且有統(tǒng)計(jì)學(xué)意義(P0.05)。術(shù)后1、3、6個(gè)月GSRe、GSRivr均較術(shù)前增加,且逐漸提高,有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論二維斑點(diǎn)追蹤技術(shù)能定量評(píng)價(jià)CABG術(shù)后患者的左心室功能,并較常規(guī)超聲心動(dòng)圖檢查更早發(fā)現(xiàn)左室收縮及舒張功能的變化,為臨床評(píng)價(jià)CABG術(shù)治療效果提供了一種較準(zhǔn)確的方法。
[Abstract]:Objective to evaluate the changes of left ventricular function in patients with coronary artery bypass grafting (CABG) by two-dimensional speckle tracking technique. Methods Echocardiography was performed in 30 patients with CABG before, 1, 3 and 6 months after operation. The long axial strain and strain rate of left ventricular two-chamber heart, three-chamber heart and four-chamber heart section were measured. The changes of left ventricular long axis systolic strain (GLSs) and strain rate (GLSRs), early diastolic strain rate (GSRe), isoelastic diastolic strain rate (GSRivr), were compared with those of conventional echocardiography and two-dimensional strain parameters after coronary artery bypass grafting (CABG).
