【摘要】：目的比較2種軟組織重建術式重建橈尺遠側(cè)關節(jié)穩(wěn)定性的生物力學效果,為臨床應用提供依據(jù)。方法選取8具成人上肢標本,通過制作損傷模型、模擬手術方式,比較正常組、損傷組、掌長肌腱組、橈側(cè)腕屈肌組分別在腕背伸30°位、腕中立位、腕掌屈30°位時旋前、旋后時扭矩的變化。結(jié)果損傷組和正常組比較的旋前、旋后扭矩差異均有統(tǒng)計學意義(P0.05);掌長肌腱組和正常組比較的差異無統(tǒng)計學意義;橈側(cè)腕屈肌組在腕關節(jié)背伸30°、腕中立位時和正常組比較的旋前、旋后扭矩差異均無統(tǒng)計學意義,而在腕關節(jié)掌屈30°時的扭矩差異有統(tǒng)計學意義(P0.05)。結(jié)論橈尺韌帶損傷會造成橈尺遠側(cè)關節(jié)不穩(wěn),通過掌長肌腱移植重建后可恢復關節(jié)穩(wěn)定性,而經(jīng)過部分橈側(cè)腕屈肌重建后,其在腕背伸30°和腕中立位時的穩(wěn)定性得到恢復,但在腕掌屈30°時仍存在關節(jié)不穩(wěn)。
[Abstract]:Objective to compare the biomechanical effects of two types of soft tissue reconstruction for the reconstruction of distal radioulnar joint. Methods 8 adult upper limb specimens were selected and compared with normal group, injury group, palmar longus tendon group, flexor Carpi radialis group in 30 擄position, neutral position of wrist, 30 擄position of wrist palmar flexion, and 30 擄position of wrist flexion. The change in torque during rotation. Results there were significant differences in pronation torque and pronation torque between the injured group and the normal group (P0.05), but there was no significant difference between the palmar longus tendon group and the normal group. There was no significant difference in the torque of pronation and supination between the flexor carpal flexor group and the normal group at 30 擄dorsal extension and wrist neutral position, but the torque difference was significant at 30 擄wrist metacarpal flexion (P0.05). Conclusion the injury of the radial ulnar ligament can cause instability of the distal radioulnar joint, and the stability of the joint can be restored after the reconstruction of the palmar longus tendon, and the stability of the flexor Carpi flexor muscle at 30 擄dorsal extension of the wrist and the neutral position of the wrist can be recovered after partial reconstruction of the flexor Carpi radialis muscle. But there is still instability at 30 擄of wrist flexion.
