【摘要】：目的建立髖臼前壁骨折模型,比較4種內(nèi)固定方式的生物力學(xué)穩(wěn)定性。方法選取20個(gè)防腐成人半骨盆標(biāo)本,建立髖臼前壁骨折模型。將骨折模型隨機(jī)分為4組,A組采用真骨盆上緣重建接骨板固定,B組采用真骨盆內(nèi)緣重建接骨板固定,C組采用順行拉力螺釘固定,D組采用逆行拉力螺釘固定。對(duì)各組標(biāo)本進(jìn)行軸向壓縮實(shí)驗(yàn),測(cè)量骨折斷端水平位移,計(jì)算剪切剛度,比較4組不同內(nèi)固定方式的穩(wěn)定性。結(jié)果相同加載載荷下,骨折斷端水平位移:A組B組C組D組,內(nèi)固定剪切剛度:D組C組B組A組。載荷為400 N時(shí)各組水平位移及內(nèi)固定剪切剛度差異均無(wú)統(tǒng)計(jì)學(xué)意義(P0.05),載荷為800 N至2000 N時(shí),除C組與D組間水平位移及內(nèi)固定剪切剛度差異無(wú)統(tǒng)計(jì)學(xué)意義外(P0.05),其余各組間比較差異均有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論髖臼前壁骨折模型可用于髖臼前壁骨折的生物力學(xué)分析,采用拉力螺釘固定髖臼前壁骨折的穩(wěn)定性高于重建接骨板固定方式,其中順行拉力螺釘固定與逆行拉力螺釘固定的穩(wěn)定性相似。
[Abstract]:Objective to establish acetabular anterior wall fracture model and compare the biomechanical stability of four internal fixation methods. Methods 20 antiseptic adult hemipelvis specimens were selected to establish acetabular anterior wall fracture model. The fracture model was randomly divided into 4 groups: group A was fixed with the reconstruction plate of the upper edge of the true pelvis, group B with the internal margin of the true pelvis, group C with the anterograde pull screw fixation, group D with the retrograde screw fixation. The axial compression experiments were carried out to measure the horizontal displacement at the fracture end, to calculate the shear stiffness, and to compare the stability of the four groups with different internal fixation methods. Results under the same loading load, the horizontal displacement of fracture end: group A, group B, group C, group D, internal fixation shear stiffness, group D, group C, group B, group A. When the load was 400N, there was no significant difference in the horizontal displacement and the shear stiffness of the internal fixation (P0.05), but the load ranged from 800N to 2000 N. There was no significant difference in horizontal displacement and internal fixation shear stiffness between group C and group D (P0.05), but there were significant differences among other groups (P0.05). Conclusion the model of acetabular anterior wall fracture can be used for biomechanical analysis of acetabular anterior wall fracture. The stability of anterograde lag screw fixation was similar to that of retrograde lag screw fixation.
