基于微型壓電與光譜生化分析系統(tǒng)的POCT新技術(shù)研究

【摘要】:微納技術(shù)推動(dòng)醫(yī)學(xué)檢驗(yàn)儀器向著微型化、自動(dòng)化方向發(fā)展,促進(jìn)了即時(shí)檢驗(yàn)的出現(xiàn)。即時(shí)檢驗(yàn)技術(shù)(POCT)具有簡(jiǎn)便快速、檢測(cè)結(jié)果及時(shí)可靠等特點(diǎn),,為疾病診斷提供了有效的輔助手段。POCT除了應(yīng)用于臨床診斷外,在現(xiàn)場(chǎng)急救、個(gè)人自檢、社區(qū)醫(yī)療等領(lǐng)域也有廣闊應(yīng)用前景,正成為醫(yī)學(xué)檢驗(yàn)的重要發(fā)展方向。 首先本論文針對(duì)腎病、糖尿病診斷指標(biāo)——尿微量蛋白的即時(shí)檢驗(yàn),采用微型壓電生化分析系統(tǒng),分別基于納米金、普魯士藍(lán)的信號(hào)放大建立了相應(yīng)的快速分析方法。其中,利用納米金信號(hào)放大分別測(cè)定了尿液中微量的免疫球蛋白G(IgG)、白蛋白(Alb)、α1-微球蛋白(α1-MG)、β2-微球蛋白(β2-MG),其檢測(cè)范圍(μg/ml)分別為0-120、2-50、0.5-40和0.09-5;檢出限(μg/ml)分別為0.84、1.54、0.98和0.085,檢測(cè)時(shí)間均為25min;利用普魯士藍(lán)放大測(cè)定了尿液中微量IgG,檢測(cè)范圍為0-100μg/ml,檢出限為0.38μg/ml,檢測(cè)時(shí)間為25min。以上兩種方法與免疫比濁法對(duì)照,應(yīng)用于臨床樣品測(cè)定,t檢驗(yàn)結(jié)果表明無(wú)顯著性差異,均能滿足醫(yī)學(xué)檢驗(yàn)要求。 其次本論文針對(duì)現(xiàn)場(chǎng)急救生化指標(biāo)——Na~+、K~+、Cl~-、CO_2、CRE、Glu、Hb的即時(shí)檢驗(yàn),采用微型光譜生化分析系統(tǒng),利用連續(xù)光譜測(cè)量實(shí)現(xiàn)了七項(xiàng)指標(biāo)的同時(shí)、快速測(cè)定。其檢測(cè)波長(zhǎng)(nm)分別為620、620、480、405、600、505、540;檢測(cè)范圍(mmol/L)分別為0-200、0-8.0、0-120、0-75、0-1.15、0-20、0-3.0;檢測(cè)時(shí)間為10min。以Na~+、Glu、Hb三項(xiàng)指標(biāo)與全自動(dòng)生化分析儀(奧林巴斯AU2700)對(duì)照,應(yīng)用于臨床樣品測(cè)定,t檢驗(yàn)結(jié)果表明無(wú)顯著性差異,均能滿足醫(yī)學(xué)檢驗(yàn)要求。 本論文采用微型化壓電與光學(xué)生化分析系統(tǒng),實(shí)現(xiàn)了腎病、糖尿病診斷指標(biāo)及現(xiàn)場(chǎng)急救生化指標(biāo)的快速檢測(cè),臨床應(yīng)用展示了由此建立新型即時(shí)檢驗(yàn)技術(shù)的可行性,對(duì)于現(xiàn)場(chǎng)急救、個(gè)人自檢、社區(qū)醫(yī)療等具有重要的應(yīng)用價(jià)值。
[Abstract]:In addition to clinical diagnosis , POCT has broad application prospect in field first aid , personal self - test , community medical treatment and so on , which is becoming the important direction of medical examination .
In this paper , the rapid analysis method was established based on the signal amplification of nano - gold and pralumin blue . The detection range ( 渭g / ml ) was 0 - 120 , 2 - 50 , 0.5 -40 and 0.09 - 5 , respectively .
The detection limit ( 渭g / ml ) was 0.84 , 1.54 , 0.98 and 0.085 , respectively , and the detection time was 25 min .
The detection limit was 0 - 100 渭g / ml , the detection limit was 0 . 38 渭g / ml and the detection time was 25 min . The results of t - test showed no significant difference , which could meet the requirement of medical examination .
In this paper , a micro - spectrum biochemical analysis system was used to detect the biochemical indexes of Na ~ + , K ~ + , Cl ~ - , CO _ 2 , CRE , Glu and Hb in the field first - aid biochemistry index . The detection wavelength ( nm ) was 620 , 620 , 480 , 405 , 600 , 505 , 540 , respectively .

In this paper , the micro - type piezoelectric and optical biochemical analysis system is used to realize the rapid detection of renal disease , diabetes diagnosis index and on - site emergency biochemistry index . The clinical application shows the feasibility of establishing a new instant test technique , and has important application value for field first aid , personal self - test , community medical treatment and the like .
