基于X線片定制股骨柄假體的設(shè)計原理及方法

【摘要】:目的介紹自編的基于X線片設(shè)計定制型股骨柄假體程序系統(tǒng)的設(shè)計原理及功能模塊。方法利用圖像處理方法提取X線片上一些患者股骨解剖特征點(diǎn)(如小粗隆最高點(diǎn)、股骨頭中心及轉(zhuǎn)子間窩等)、2個規(guī)劃的手術(shù)截骨位置點(diǎn)(大粗隆側(cè)截骨邊界位置點(diǎn)及截骨線最低點(diǎn)位置點(diǎn))以及一些設(shè)計用關(guān)鍵位置點(diǎn)(如假體遠(yuǎn)端位置、假體頸干連接段最高點(diǎn)位置等),以此預(yù)測患者股骨近端髓腔,并通過輸入設(shè)計參數(shù)值(如頸干角、前傾角、預(yù)留的松質(zhì)骨厚度、假體匹配段截面圓弧半徑及假體過渡段高度等)設(shè)計出定制型股骨柄假體。并對設(shè)計出的定制股骨柄假體進(jìn)行匹配區(qū)域多個截面的二維截面驗(yàn)證和三維的總體驗(yàn)證。結(jié)果根據(jù)驗(yàn)證情況,調(diào)整設(shè)計參數(shù)重新生成股骨柄假體模型點(diǎn)云數(shù)據(jù),使設(shè)計出的股骨柄假體與患者股骨腔相匹配,達(dá)到定制型假體設(shè)計要求。結(jié)論介紹的程序系統(tǒng)可用于為患者設(shè)計定制型股骨柄假體,設(shè)計周期較短,成本較低,有利于促進(jìn)定制型股骨柄假體在臨床上的應(yīng)用,進(jìn)一步提高患者的生活質(zhì)量。
[Abstract]:Objective to introduce the design principle and function module of customized femoral handle prosthesis program system based on X-ray film. The center of the femoral head and the intertrochanteric fossa, two planned osteotomy sites (the great trochanter side osteotomy boundary and the lowest point of the osteotomy line), and some design key points (such as the distal position of the prosthesis), In order to predict the proximal medullary cavity of the femur, and input the design parameters (such as neck shaft angle, anteversion angle, reserved thickness of cancellous bone, etc.). The section arc radius and the height of the prosthetic transition segment are used to design the customized femur handle prosthesis. The designed customized femur handle prosthesis is verified by two-dimensional cross-section and three-dimensional experience of multiple cross-sections in the matching region. The results are based on the verification, Adjusting the design parameters to reconstruct the point cloud data of the femoral prosthesis model to match the designed femoral pedicle prosthesis with the femoral cavity of the patient. Conclusion the introduced program system can be used to design customized femoral handle prosthesis for patients with short design period and low cost, which is helpful to promote the clinical application of customized femoral handle prosthesis. Further improve the quality of life of patients.
