有限元法分析髖關(guān)節(jié)表面置換后髖關(guān)節(jié)的力學(xué)特點(diǎn)

【摘要】:背景:有限元分析在全髖關(guān)節(jié)置換中的研究比較多,對(duì)髖關(guān)節(jié)表面置換后生物力學(xué)分布特點(diǎn)的研究較少。目的:三維有限元法分析髖關(guān)節(jié)表面置換后髖關(guān)節(jié)的生物力學(xué)特點(diǎn)。方法:建立金屬對(duì)金屬髖關(guān)節(jié)表面置換后的三維有限元模型,在髖臼側(cè)加載與重力線平行的350N載荷,觀察假體應(yīng)力分布、股骨應(yīng)力分布和股骨頭不同區(qū)域應(yīng)力值。結(jié)果與結(jié)論:①髖臼杯沿和髖臼主體的應(yīng)力值分別為(0.63±0.34)MPa和(1.89±0.67)MPa,髖臼杯沿的應(yīng)力值高于髖臼主體(P0.05);②應(yīng)力集中在股骨頸,股骨頭和股骨頸交界處出現(xiàn)應(yīng)力遮擋,股骨假體柄周圍骨質(zhì)的應(yīng)力值為(158.24±28.79)MPa,股骨假體柄應(yīng)力值為(186.46±22.37)MPa,股骨假體柄周圍骨質(zhì)和股骨假體柄的應(yīng)力值比較差異無顯著性意義(P0.05);③股骨頭分區(qū)8個(gè)區(qū)域之間應(yīng)力值差異有顯著性意義(P0.05),區(qū)域6和區(qū)域8的應(yīng)力值高于其他區(qū)域(P0.05);④表明金屬對(duì)金屬髖關(guān)節(jié)表面置換后股骨頸應(yīng)力集中,股骨假體側(cè)應(yīng)力遮擋,股骨頸內(nèi)側(cè)、股骨頸假體和骨質(zhì)交界處的應(yīng)力較大。
[Abstract]:Background: finite element analysis (FEM) has been widely used in total hip arthroplasty. Few studies have been done on the biomechanical characteristics of the hip joint after surface replacement. Objective: to analyze the biomechanical characteristics of the hip joint after the surface replacement by three dimensional finite element method. Methods: to establish the metal to metal surface replacement of the hip joint. After the 3D finite element model, The stress distribution of the prosthesis was observed by loading 350 N load parallel to the gravity line on the acetabular side. Results and conclusion the stress values of the acetabular cup edge and the acetabular body were 0.63 鹵0.34 MPA and 1.89 鹵0.67 MPA, respectively. The stress values of the acetabular cup edge were higher than that of the acetabular body (P 0.05). Stress occlusion occurs at the junction of the femoral head and the femoral neck. The stress value of periprosthetic bone was 158.24 鹵28.79 MPA, and that of femoral prosthesis was 186.46 鹵22.37 MPA. There was no significant difference in stress between the surrounding bone of femoral prosthesis and femoral prosthesis. The stress values in region 6 and region 8 were higher than those in other regions, indicating that the stress concentration of the femoral neck after metal hip surface replacement was higher than that in other regions. The stress is larger in the medial femoral neck and at the junction of femoral neck prosthesis and bone.
