【摘要】：中醫(yī)腰椎手法在腰痛等慢性損傷性腰椎病的疼痛治療中有明顯的臨床優(yōu)勢(shì),但其基礎(chǔ)研究不足是限制手法進(jìn)步的重要因素。利用有限元技術(shù)能夠很好地模擬各類腰椎手法的力學(xué)狀況,分析其作用機(jī)制,驗(yàn)證假說(shuō),規(guī)范手法操作,定量、定性和優(yōu)化治療方案,為手法治療的基礎(chǔ)研究提供有效的研究方法。通過(guò)回顧近年腰椎手法治療的有限元研究,探討不同腰椎手法對(duì)椎間盤(pán)、附屬結(jié)構(gòu)、脊柱負(fù)荷以及椎體力學(xué)穩(wěn)定性的影響,結(jié)果發(fā)現(xiàn)目前關(guān)于腰椎手法的有限元研究有待于對(duì)模擬方法進(jìn)行標(biāo)準(zhǔn)化和精確化,同時(shí)可進(jìn)一步推廣有限元的研究思路,更好地指導(dǎo)中醫(yī)腰椎手法的臨床運(yùn)用。
[Abstract]:Lumbar manipulation in traditional Chinese medicine has obvious clinical advantages in the treatment of pain of chronic injured lumbar vertebrae, such as low back pain. However, the lack of basic research is an important factor limiting the progress of manipulation. The finite element technique can well simulate the mechanical status of all kinds of lumbar manipulations, analyze its mechanism of action, verify the hypothesis, standardize the manipulation, and quantify it. The qualitative and optimized treatment schemes provide an effective method for the basic research of manipulation therapy. By reviewing the finite element study of lumbar manipulation therapy in recent years, we discuss the intervertebral disc and accessory structure of different lumbar manipulations. The effects of spinal load and mechanical stability of vertebrae. The results show that the present finite element study on lumbar manipulations needs to be standardized and accurate, and can be further extended to the idea of finite element analysis. Better guide the clinical application of lumbar manipulation of traditional Chinese medicine.
