IL-21、CCL20在扁平苔癬皮損中的表達(dá)及意義

【摘要】：目的:探討扁平苔癬皮損中白介素-21(IL-21)和CC趨化因子配體20(CCL20)的表達(dá)及意義。方法:病例組:30例,均為2015年-2016年就診于山大一院皮膚科門診,并且經(jīng)臨床和病理確診為扁平苔蘚的患者,其中男16例,女14例,年齡18-60歲,平均年齡41.73歲,半年內(nèi)均未接受過糖皮質(zhì)激素、免疫抑制劑、免疫調(diào)節(jié)劑治療,且排除其他全身系統(tǒng)性疾病、嚴(yán)重肝病、腎病、自身免疫性疾病。對照組:30例,均為同期就診于山大一院整形科的健康人,其中男13例,女17例,年齡20-65歲,平均年齡43.3歲,均無任何皮膚病和系統(tǒng)性疾病。兩組成員的性別和年齡比較差異均無統(tǒng)計學(xué)意義(P0.05),具有可比性。所有組織均經(jīng)甲醛固定石蠟包埋備用,IL-21、CCL20在扁平苔蘚皮損中的表達(dá)水平采用免疫組織化學(xué)方法測定。運用SPSS17.0軟件來對最后的實驗結(jié)果進(jìn)行分析,采用兩獨立樣本t檢驗的方法來比較兩組,Pearson相關(guān)分析用來分析指標(biāo)間的相關(guān)性,差異有統(tǒng)計學(xué)意義的評判標(biāo)準(zhǔn)為P0.05。結(jié)果:IL-21和CCL20的表達(dá)水平均用灰度值大小來表示。IL-21在扁平苔蘚患者皮損中的表達(dá)水平為(189.19±15.42),在健康對照組中的表達(dá)水平為(232.55±12.97)。CCL20在扁平苔蘚皮損中的表達(dá)為(198.91±18.65),在健康對照組的表達(dá)水平為(225.68±8.72)。統(tǒng)計分析結(jié)果顯示,IL-21、CCL20在扁平苔蘚患者皮損中的表達(dá)水平較健康對照組升高(t分別為10.532,6.112),差異比較均有統(tǒng)計學(xué)意義(均P0.001)。Pearson相關(guān)分析表明在扁平苔蘚患者皮損中IL-21、CCL20的表達(dá)成正相關(guān)(r=0.691,P0.001)。結(jié)論:IL-21、CCL20在扁平苔蘚患者皮損中的表達(dá)水平高于健康對照組(P0.001),且二者在扁平苔蘚皮損中的表達(dá)水平成正相關(guān)性(P0.001),提示IL-21、CCL20可能在扁平苔蘚的發(fā)病中發(fā)揮一定的作用,同時可能為扁平苔蘚的治療提供了新的理論依據(jù)。
[Abstract]:Aim: to investigate the expression and significance of interleukin-21 (IL-21) and CC chemokine ligand 20 (CCL20) in the lesions of lichen planus. Methods: a total of 30 patients with lichen planus, including 16 males and 14 females, aged 18-60 years, with an average age of 41.73 years, were admitted to the dermatology clinic of the first Hospital of Shandong Province from 2015 to 2016. No glucocorticoids, immunosuppressants, immunomodulators were used in six months, and other systemic diseases, severe liver diseases, nephropathy, autoimmune diseases were excluded. The control group consisted of 30 healthy persons, including 13 males and 17 females, aged 20-65 years, with an average age of 43.3 years, all of them without any skin diseases and systemic diseases. There was no significant difference in sex and age between the two groups (P0.05), which was comparable. The expression of IL-21,CCL20 in lichen planus was determined by immunohistochemical method. SPSS17.0 software was used to analyze the final experimental results, and two independent samples t-test method was used to compare the two groups. Pearson correlation analysis was used to analyze the correlation between the indicators. The difference was statistically significant (P0.05). Results: the expression levels of IL-21 and CCL20 were expressed in gray scale. The expression level of IL-21 in the lesions of lichen planus was (189.19 鹵15.42). The expression of CCL20 was (198.91 鹵18.65) in lichen planus and (225.68 鹵8.72) in healthy control. The results of statistical analysis showed that the expression of IL-21,CCL20 in the lesions of lichen planus patients was higher than that in healthy controls (t = 10.532 鹵6.112, respectively). The difference was statistically significant (P0. 001). Pearson correlation analysis showed that the expression of IL-21,CCL20 was positively correlated in the lesions of patients with lichen planus (r 0. 691P 0. 001). Conclusion: the expression of IL-21,CCL20 in patients with lichen planus is higher than that in healthy controls (P0. 001), and there is a positive correlation between the expression of IL-21,CCL20 and Lichen planus (P0. 001), which suggests that IL-21, is expressed in patients with Lichen planus (P0. 001). CCL20 may play a role in the pathogenesis of lichen planus and may provide a new theoretical basis for the treatment of lichen planus.
