【摘要】：目的：本實(shí)驗(yàn)通過(guò)對(duì)進(jìn)行期尋常型銀屑病皮損組織、非皮損組織及正常人皮膚組織中防御素-2(HBD-2)、防御素-3(HBD-3)及腫瘤壞死因子（TNF-α）表達(dá)情況的研究，探討HBD-2，-3與TNF-α之間的關(guān)系；三者表達(dá)水平與銀屑病嚴(yán)重程度之間的關(guān)系，進(jìn)一步揭示HBD在尋常型銀屑病發(fā)生發(fā)展過(guò)程中的作用機(jī)制。 方法： 1.收集45例進(jìn)行期尋常型銀屑病患者的皮損組織、非皮損組織及15例正常人皮膚組織,采用實(shí)時(shí)熒光定量逆轉(zhuǎn)錄聚合酶鏈反應(yīng)法(RT-PCR)檢測(cè)標(biāo)本中HBD-2，-3及TNF-α的mRNA表達(dá)水平。比較不同組別間三者表達(dá)的差異，分析HBD-2與TNF-α，HBD-3與TNF-α表達(dá)的相關(guān)性。 2.將45例進(jìn)行期尋常型銀屑病患者按照PASI評(píng)分分為輕、中、重三組，，其中輕度組25例，中度組12例，重度組8例。采用RT-PCR檢測(cè)標(biāo)本中HBD-2，-3及TNF-α的mRNA表達(dá)水平。比較不同組別間三者表達(dá)的差異，分析三者與銀屑病PASI評(píng)分的相關(guān)性。 結(jié)果： 1.HBD-2，-3及TNF-αmRNA在正常對(duì)照組、非皮損組及皮損組中均有表達(dá)，且表達(dá)依次升高。HBD-2、HBD-3及TNF-α的mRNA在皮損組中的表達(dá)顯著高于非皮損組（P＜0.01）和正常對(duì)照組（P＜0.01）；在非皮損組中的表達(dá)又顯著高于正常對(duì)照組（P＜0.01）。 2.在皮損組中HBD-2與TNF-α，HBD-3與TNF-α的表達(dá)均呈正相關(guān)，相關(guān)系數(shù)分別為r=0.807、r=0.809；（P值均＜0.05）。 3.HBD-2，-3及TNF-αmRNA在銀屑病皮損重度組中的表達(dá)顯著高于中度組（P＜0.01）和輕度組（P＜0.01）；在中度組中的表達(dá)又顯著高于輕度組（P＜0.01）。 4.HBD-2、HBD-3及TNF-α的mRNA在皮損組中的表達(dá)均與患者PASI評(píng)分成正相關(guān)，相關(guān)系數(shù)分別為r=0.887、r=0.756、r=0.785；（P值均＜0.05）。結(jié)論：尋常型銀屑病皮損中HBD-2，-3及TNF-αmRNA的表達(dá)與銀屑病PASI評(píng)分正相關(guān)。HBD-2，-3與TNF-α的表達(dá)之間也存在正相關(guān)。我們推測(cè)三者之間可能存在相互作用相互誘生的關(guān)系。HBD-2、HBD-3的表達(dá)增高是一把雙刃劍在防御感染的同時(shí)，也可能擴(kuò)大了炎反應(yīng)，與TNF-α一起參與了銀屑病的發(fā)病過(guò)程。
[Abstract]:Objective: to investigate the effects of defensin-2 (HBD-2) in psoriasis vulgaris, noninvasive tissues and normal skin tissues in patients with psoriasis vulgaris. To investigate the expression of defensin-3 (HBD-3) and tumor necrosis factor 偽 (TNF- 偽) and to explore the relationship between HBD-2,-3 and TNF- 偽. The relationship between the three levels of expression and the severity of psoriasis further revealed the role of HBD in the pathogenesis of psoriasis vulgaris. Methods: 1. HBD-2, in 45 patients with psoriasis vulgaris was detected by real-time quantitative reverse transcriptase polymerase chain reaction (RT-PCR). MRNA expression level of-3 and TNF- 偽. The expression of HBD-2 and TNF- 偽, HBD-3 and TNF- 偽 were compared among different groups. 2. According to PASI score, 45 patients with psoriasis vulgaris were divided into mild, moderate and severe groups, including 25 mild, 12 moderate and 8 severe psoriasis. The mRNA expression of HBD-2,-3 and TNF- 偽 was detected by RT-PCR. The differences of expression among different groups were compared and the correlation between the three groups and PASI score of psoriasis was analyzed. Results: 1. HBD-2n-3 and TNF- 偽 mRNA were expressed in normal control group, non-lesion group and skin lesion group, and the expression of HBD-2n-3 and TNF- 偽 mRNA increased in turn. The expression of mRNA of HBD-3 and TNF- 偽 in the lesion group was significantly higher than that in the non-lesion group (P < 0. 01) and the normal control group (P < 0. 01). The expression in non-lesion group was significantly higher than that in normal control group (P < 0. 01). 2. There was a positive correlation between the expression of HBD-2 and TNF- 偽, HBD-3 and TNF- 偽 in the lesion group, and the correlation coefficients were 0.807 and 0.809, respectively (P < 0. 05). 3. The expression of HBD-2n-3 and TNF- 偽 mRNA in severe psoriatic lesions was significantly higher than that in moderate and mild psoriatic lesions (P < 0. 01), and was significantly higher in moderate group than in mild group (P < 0. 01). 4. The expression of HBD-3 and TNF- 偽 mRNA in the lesions was positively correlated with the PASI score, and the correlation coefficients were 0.756 and 0.756, respectively (P < 0. 05). Conclusion: the expression of HBD-2,-3 and TNF- 偽 mRNA in psoriatic lesions is positively correlated with the PASI score of psoriasis, and there is a positive correlation between the expression of HBD-2,-3 and TNF- 偽 in psoriasis vulgaris. We speculate that there may be interaction and interaction between the three. The increased expression of HBD-2,HBD-3 is a double-edged sword that may expand the inflammatory response while defending against infection. TNF- 偽 is involved in the pathogenesis of psoriasis.
