【摘要】：患者女,20歲。四肢丘疹、丘膿皰疹、潰瘍5年,頸部結(jié)節(jié)、潰瘍、瘢痕6個(gè)月。相關(guān)實(shí)驗(yàn)室檢查包括結(jié)核PPD試驗(yàn)(強(qiáng)陽(yáng)性),γ-干擾素釋放試驗(yàn)(陽(yáng)性),結(jié)核熒光定量PCR(可疑陽(yáng)性)。頸部皮損組織病理示:表皮輕度肥厚,真皮內(nèi)大片干酪樣壞死,可見(jiàn)散在上皮樣肉芽腫及較多淋巴細(xì)胞、少量漿細(xì)胞浸潤(rùn)。小腿皮損組織病理示:表皮呈銀屑病樣增生,真皮及皮下脂肪淺層小血管周圍較多淋巴細(xì)胞,少量嗜酸性粒細(xì)胞浸潤(rùn)。診斷:丘疹壞死性結(jié)核疹并瘰疬性皮膚結(jié)核。
[Abstract]:The patient was 20 years old. Extremities papules, ulcers for 5 years, neck nodules, ulcers, scars for 6 months. Laboratory tests included tuberculosis PPD test (strong positive), interferon 緯 release test (positive) and fluorescent quantitative PCR (suspicious positive). The histopathology of the cervical lesions showed that the epidermis was slightly hypertrophic, the dermis was large and caseous necrosis was found, the epithelioid granuloma and more lymphocytes were scattered, and a small amount of plasma cells were infiltrated. The histopathology of leg lesions showed that epidermis proliferated like psoriasis more lymphocytes and a few eosinophilic granulocytes infiltrated around small vessels of dermis and subcutaneous fat. Diagnosis: papular necrotic tuberculosis with scrofula skin tuberculosis.
