【摘要】：目的分析肛門直腸梅毒的臨床及組織病理學(xué)特點(diǎn),提高臨床及病理醫(yī)生對梅毒的認(rèn)識,避免漏診、誤診。方法回顧性分析5例肛門直腸梅毒的臨床及組織病理學(xué)形態(tài),結(jié)合實(shí)驗(yàn)室資料進(jìn)行診斷,并分析誤診原因。結(jié)果患者均為男性,均有不潔性生活史。其中一期梅毒1例,二期3例,三期1例,初診時(shí)臨床、病理均漏診。組織病理學(xué)特點(diǎn):肛周皮膚、肛管、直腸黏膜的表面輕度糜爛,大量中性粒細(xì)胞浸潤,微膿腫形成;鱗狀上皮假上皮瘤樣增生,棘層增厚,上皮腳下延,表皮細(xì)胞內(nèi)及細(xì)胞間水腫,真皮層大量漿細(xì)胞及淋巴細(xì)胞浸潤,血管周圍尤為顯著。結(jié)論肛門、直腸梅毒臨床少見,臨床和病理醫(yī)生容易漏診、誤診。掌握梅毒的臨床特點(diǎn)和組織病理形態(tài)學(xué)特征,提高對梅毒的認(rèn)識。
[Abstract]:Objective to analyze the clinical and histopathological features of anorectal syphilis, to improve the understanding of syphilis among clinicians and pathologists, to avoid missed diagnosis and misdiagnosis. Methods the clinical and histopathological features of 5 cases of anorectal syphilis were analyzed retrospectively. Results all the patients were male and had a history of unclean sexual life. There were 1 case of primary syphilis, 3 cases of secondary syphilis and 1 case of stage 3 syphilis. Histopathological features: perianal skin, anal canal, rectal mucosa surface mild erosion, a large number of neutrophils infiltration, microabscess formation; Squamous epithelial pseudoepithelioma hyperplasia, thickening of spinous layer, extension of epithelium, edema in epidermal cells and between cells, infiltration of plasma cells and lymphocytes in dermis, especially around blood vessels. Conclusion anal and rectal syphilis is rare in clinic, and it is easy to be missed and misdiagnosed by clinicians and pathologists. To grasp the clinical and histopathologic features of syphilis and improve the understanding of syphilis.
