【摘要】：目的： 皮膚鱗狀細胞癌(squamous cell carcinoma, SCC)是起源于角質(zhì)形成細胞或附屬器鱗狀上皮細胞的惡性腫瘤,是最常見的皮膚惡性腫瘤之一,具有侵襲性,易于發(fā)生臨近或遠位轉(zhuǎn)移,尤其轉(zhuǎn)移到重要的器官,從而危及生命。該病的發(fā)生、侵襲和轉(zhuǎn)移機制目前仍不十分清楚,因此研究皮膚鱗狀細胞癌侵襲和轉(zhuǎn)移的機理,對于腫瘤的預防和治療具有重要意義。 埃茲蛋白(Ezrin protein, Ezrin)是將細胞膜蛋白與肌動蛋白-細胞骨架連接起來的橋接蛋白,具有維持細胞形態(tài)和運動、連接黏附分子及調(diào)節(jié)信號轉(zhuǎn)導等功能。目前研究發(fā)現(xiàn),其主要通過與腫瘤細胞表面黏附分子發(fā)生作用來影響腫瘤的分化、侵襲性和轉(zhuǎn)移；水通道蛋白(aquaporins, AQPs)是一類能快速轉(zhuǎn)運水的膜整合蛋白,皮膚上主要分布的水通道蛋白是水通道蛋白3 (aquaporin-3,AQP3)。目前發(fā)現(xiàn)AQP3參與多種生理病理過程,其高表達與惡性腫瘤的侵襲和轉(zhuǎn)移有著密切的聯(lián)系。 檢測Ezrin蛋白,AQP3在皮膚鱗狀細胞癌中的表達,探討它們在皮膚鱗狀細胞癌發(fā)生、發(fā)展中的意義及其與臨床病理特征的相關性,并探討二者的相互關系以及在皮膚鱗狀細胞癌發(fā)病中所起的作用,以期為皮膚鱗狀細胞癌的發(fā)病機制提供理論依據(jù)。 材料與方法： 收集石蠟包埋的皮膚鱗狀細胞癌標本43例,其中高分化16例,中分化15例,低分化12例。對照組為15例正常皮膚。應用免疫組化SP法檢測43例皮膚鱗狀細胞癌和15例正常皮膚組織中Ezrin蛋白和AQP3的表達水平,用已知的Ezrin和AQP3陽性的皮膚鱗狀細胞癌標本做為陽性對照,以PBS代替一抗作為陰性對照；結(jié)合臨床病理學資料應用SPSS13.0統(tǒng)計軟件包進行統(tǒng)計學分析。 結(jié)果： Ezrin蛋白在43例皮膚鱗狀細胞癌中陽性率為93%,15例正常皮膚組織中Ezrin蛋白陽性表達率為6.67%,兩者比較差異有顯著性(P0.01);Ezrin蛋白的表達與腫瘤的分化程度有顯著相關性,其陽性率隨皮膚鱗狀細胞癌病理分級的降低而升高,差異有顯著性(P0.01)；Ezrin蛋白在皮膚鱗狀細胞癌中則以細胞質(zhì)染色或細胞膜、細胞質(zhì)均著色為主。AQP3在皮膚鱗狀細胞癌和正常皮膚中的陽性表達率分別為88.4%和6.67%,差異有顯著性(P0.01)；在皮膚鱗狀細胞癌中AQP3蛋白的表達與腫瘤的分化程度有顯著相關性(P0.01),即隨皮膚鱗狀細胞癌病理分級的增高而升高；AQP3在皮膚鱗狀細胞癌中則以細胞質(zhì)染色或細胞膜、細胞質(zhì)均著色為主。Ezrin蛋白和AQP3的表達呈顯著正相關,有統(tǒng)計學意義(r=0.573,P0.01)。 結(jié)論： 1、Ezrin蛋白在皮膚鱗狀細胞癌中表達增高,且表達水平在皮膚鱗狀細胞癌中與腫瘤的分化程度呈負相關,即分化越差,表達越高。 2、AQP3在皮膚鱗狀細胞癌中表達增高,且表達水平在皮膚鱗狀細胞癌中與腫瘤的分化程度呈正相關,即分化越高,表達越高。 3、Ezrin蛋白和AQP3在皮膚鱗狀細胞癌中的表達呈正相關,表明二者在皮膚鱗狀細胞癌的發(fā)生、發(fā)展過程中可能具有協(xié)同作用。
