【摘要】：目的:通過(guò)對(duì)沈陽(yáng)市第七人民醫(yī)院皮膚科近2年來(lái)泛發(fā)性膿皰型銀屑病患者病歷進(jìn)行回顧性分析,總結(jié)泛發(fā)性膿皰型銀屑病的一般發(fā)病情況及治療經(jīng)驗(yàn),加強(qiáng)對(duì)本病的認(rèn)識(shí),為今后預(yù)防和治療提供參考依據(jù)。材料與方法:回顧性調(diào)查2014年2月-2016年11月于沈陽(yáng)市第七人民醫(yī)院皮膚科住院治療的泛發(fā)性膿皰型銀屑病患者的住院病歷,對(duì)其性別、年齡、病程、發(fā)病季節(jié)、誘因、疾病史、家族史、生活習(xí)慣、皮膚表現(xiàn)、特殊受累、伴隨癥狀、實(shí)驗(yàn)室檢查、既往治療情況、住院天數(shù)、住院期間用藥情況、療效評(píng)價(jià)等項(xiàng)目進(jìn)行登記,應(yīng)用統(tǒng)計(jì)學(xué)方法得出相關(guān)結(jié)論。結(jié)果:1.一般情況:共182例患者,男性95例,女性87例,男女比為1.09:1。平均年齡46.55±17.92歲。平均發(fā)病年齡44.04±18.65歲。病程2天-40年不等。平均住院天數(shù)24.24±14.06天。發(fā)病多于冬春季節(jié)。2.誘因:122例本次發(fā)病無(wú)明顯誘因,感染誘發(fā)19例,藥物誘發(fā)37例,其他原因誘發(fā)4例。3.疾病史:29例患者否認(rèn)疾病史。其余病例均有1種或多種疾病。4.家族遺傳史:有尋常型銀屑病家族史5例;5.實(shí)驗(yàn)室檢查:白細(xì)胞升高82例;中性粒細(xì)胞升高90例;血小板計(jì)數(shù)升高22例;尿常規(guī)異常43例;肝功異常28例;總膽紅素升高18例;間接膽紅素升高14例;腎功異常16例;尿酸升高20例;血鈣下降29例;總蛋白、白蛋白降低98例;血脂異常57例;32例行皰液細(xì)菌培養(yǎng),14例結(jié)果為陰性;C-反應(yīng)蛋白升高57例;超敏C反應(yīng)蛋白升高26例;總Ig E陽(yáng)性67例;T細(xì)胞亞群比例紊亂24例。6.治療結(jié)果:治愈106例,顯效67例,有效1例,無(wú)效8例,總有效率為95.05%。結(jié)論:1.泛發(fā)性膿皰型銀屑病病程長(zhǎng)、易復(fù)發(fā),可發(fā)生于任何年齡階段,男女均可患病;好發(fā)于冬春季節(jié)。2.泛發(fā)性膿皰型銀屑病病因不明,誘發(fā)因素多為感染和藥物使用不當(dāng)。3.中西醫(yī)結(jié)合治療泛發(fā)性膿皰型銀屑病療效顯著,總有效率達(dá)95.05%。
[Abstract]:Objective: to analyze retrospectively the medical records of patients with generalized pustular psoriasis in the Department of Dermatology of the Seventh people's Hospital of Shenyang in recent two years, and to summarize the general incidence and treatment experience of generalized pustular psoriasis, so as to strengthen the understanding of the disease. To provide reference for prevention and treatment in the future. Materials and methods: the medical records of patients with generalized pustular psoriasis who were hospitalized in Department of Dermatology, Seventh people's Hospital of Shenyang from February 2014 to November 2016 were retrospectively investigated. Their sex, age, course of disease, onset season, inducement, history of disease were analyzed. Family history, living habits, skin manifestation, special involvement, accompanying symptoms, laboratory examination, past treatment, hospitalization days, drug use during hospitalization, curative effect evaluation and other items were registered, and the relevant conclusions were obtained by statistical method. The result is 1: 1. General: 182 patients, 95 males and 87 females, with a ratio of 1. 09: 1. The average age was 46.55 鹵17.92 years old. The mean age of onset was 44.04 鹵18.65 years old. The course of disease ranged from 2 days to 40 years. The average hospitalization days were 24.24 鹵14.06 days. The incidence was more than that in winter and spring. There were 19 cases of infection, 37 cases of drug induction and 4 cases of other causes. History of disease: 29 patients denied the history of disease. The rest had one or more diseases. Family genetic history: 5 cases with psoriasis vulgaris. Laboratory examination showed that there were 82 cases of leukocytosis, 90 cases of neutrophils, 22 cases of platelet count, 43 cases of abnormal urine routine, 28 cases of abnormal liver function, 18 cases of total bilirubin, 14 cases of indirect bilirubin, 16 cases of abnormal renal function. Uric acid increased in 20 cases, serum calcium decreased in 29 cases, total protein and albumin decreased in 98 cases, dyslipidemia in 32 cases, vesicular fluid bacterial culture in 14 cases, negative increase of C-reactive protein in 57 cases, hypersensitive C-reactive protein in 26 cases, hypolipidemia in 32 cases, hypersensitive C-reactive protein in 26 cases. Total IgE was positive in 67 cases with disorder of T cell subsets in 24 cases. Results: 106 cases were cured, 67 cases were effective, 1 case was effective and 8 cases were ineffective. The total effective rate was 95.05. Conclusion 1. Generalized pustular psoriasis has a long course of disease and is easy to recur. It can occur at any age. The etiology of generalized pustular psoriasis is unknown, and the inducing factors are infection and improper use of drugs. The curative effect of integrated traditional Chinese and western medicine on generalized pustular psoriasis is remarkable, the total effective rate is 95.05.
