【摘要】：目的測(cè)定血管活性腸肽(VIP)、白介素(IL)-5和組胺在慢性特發(fā)性蕁麻疹(CIU)患者血清中的水平,并與正常對(duì)照組進(jìn)行比較,初步探討神經(jīng)精神因素在CIU發(fā)病中的作用。方法隨機(jī)收集(2008年11月-2009年4月)在我院門診確診的CIU患者46例,隨機(jī)選擇健康志愿者34例,應(yīng)用ELISA法測(cè)定46例CIU患者及34例正常人血清VIP、IL-5和組胺的含量,采用SPSS17.0統(tǒng)計(jì)軟件包進(jìn)行統(tǒng)計(jì)學(xué)處理,CIU組和對(duì)照組血清VIP、IL-5、組胺水平統(tǒng)計(jì)推斷采用t檢驗(yàn),CIU組血清VIP、IL-5、組胺水平之間相關(guān)分析采用直線相關(guān)分析。檢驗(yàn)水準(zhǔn)α=0.05。結(jié)果CIU組血清VIP、IL-5和組胺水平均高于正常對(duì)照組(P0.05)。CIU組血清VIP與組胺水平成正相關(guān)(r=0.343,P0.05),血清VIP與IL-5(r=-0.040,P0.05)以及IL-5與組胺之間(r=-0.057,P0.05)無相關(guān)性。結(jié)論本研究結(jié)果進(jìn)一步證實(shí)組胺在CIU發(fā)病中的關(guān)鍵性作用,并且提示VIP可能通過組胺釋放而誘發(fā)或加重CIU,推測(cè)神經(jīng)精神因素在CIU發(fā)病中的機(jī)理之一可能是通VIP觸發(fā)肥大細(xì)胞脫顆粒釋放組胺誘發(fā)或加重CIU。一方面,神經(jīng)精神因素可能通過此途徑誘發(fā)CIU而另一方面,CIU又會(huì)加重患者的心理負(fù)擔(dān),刺激VIP的進(jìn)一步釋放,從而進(jìn)一步加重病情,形成惡性循環(huán),這可能是CIU慢性反復(fù)發(fā)作的原因之一。本研究提示CIU患者可能存在Th2亢進(jìn),但在CIU發(fā)病機(jī)制中,VIP是否通過影響Thl/Th2亞群失衡而發(fā)揮作用,尚需深入研究。
[Abstract]:Objective to determine the level of vasoactive intestinal peptide (VIP), interleukin (IL) -5 and histamine in the serum of patients with chronic idiopathic urticaria (CIU), and to compare with the normal control group and preliminarily explore the role of neuropsychic factors in the pathogenesis of CIU. Methods 46 patients with CIU in our hospital were randomly collected (November 2008 -2009 year April) and were randomly selected. 34 healthy volunteers were selected. The serum levels of VIP, IL-5 and histamine in 46 cases of CIU and 34 normal subjects were measured by ELISA. The serum VIP, IL-5 and histamine levels in the CIU group and the control group were determined by t test, VIP in the CIU group, IL-5, and histamine levels in the CIU group and the control group. The correlation analysis between the levels of histamine and the histamine was linear correlation. The level of serum VIP, IL-5 and histamine in group CIU was higher than that of the normal control group (P0.05).CIU group (r=0.343, P0.05), and there was no correlation between the serum VIP and IL-5 (r=-0.040, =0.05.) and histamine. Conclusion the results of this study further confirmed that histamine is in the pathogenesis. The key role in the study suggests that VIP may induce or aggravate CIU through histamine release, presumably one of the mechanisms of neuropsychic factors in the pathogenesis of CIU may be through VIP triggering mast cells degranulation and releasing histamine to induce or aggravate CIU.. Neuropsychic factors may pass through this pathway to induce CIU and on the other, CIU will aggravate. The psychological burden of patients, stimulating the further release of VIP, further aggravating the condition and forming a vicious cycle, may be one of the reasons for the chronic recurrent attacks of CIU. This study suggests that CIU patients may have Th2 hyperactivity, but in the pathogenesis of CIU, whether VIP plays a role in the imbalance of the Thl/ Th2 subgroup is still needed to be studied in depth.
