【摘要】：目的:探討皰疹樣天皰瘡的臨床表現(xiàn)、實(shí)驗(yàn)室檢查和治療特點(diǎn)。方法:對13例皰疹樣天皰瘡患者的臨床資料進(jìn)行回顧性分析。結(jié)果:13例患者中男女比例為1.6∶1,平均發(fā)病年齡(71±12)歲。大多數(shù)患者皮疹主要發(fā)生于軀干和四肢,頭面部也可累及,多無口腔黏膜累及,臨床以環(huán)狀紅斑及緊張性水皰為特點(diǎn),伴劇烈瘙癢。皮損組織病理表現(xiàn)為表皮內(nèi)水皰及嗜酸性粒細(xì)胞海綿形成。直接免疫熒光顯示棘細(xì)胞間IgG和(或)C3沉積。糖皮質(zhì)激素是治療皰疹樣天皰瘡的主要藥物,重型患者糖皮質(zhì)激素初始量、最大控制量及累積劑量均高于輕型患者。結(jié)論:皰疹樣天皰瘡是一種有特殊臨床表現(xiàn)及組織病理特點(diǎn)的自身免疫性大皰性皮病,多數(shù)患者經(jīng)糖皮質(zhì)激素治療病情控制較好。
[Abstract]:Objective: to investigate the clinical manifestation, laboratory examination and treatment of herpes pemphigus. Methods: the clinical data of 13 patients with herpes pemphigus were retrospectively analyzed. Results the ratio of male to female was 1.6: 1, and the mean age of onset was 71 鹵12 years. Most patients with rash mainly occurred in the trunk and limbs, the head and face can also be involved, most of the oral mucosal involvement, clinical characteristics of annular erythema and tense blister, with severe pruritus. The pathological features of the lesions were intraepidermal blister and eosinophil sponge formation. Direct immunofluorescence showed the deposition of IgG and / or C 3 in the intercellular spine. Glucocorticoid is the main drug in the treatment of herpetic pemphigus. The initial dose of glucocorticoid, the maximum controlled dose and the cumulative dose of glucocorticoid in severe patients are higher than those in mild patients. Conclusion: herpetic pemphigus is an autoimmune bullous dermatosis with special clinical manifestations and histopathological features. Most patients are well controlled by glucocorticoid therapy.
