【摘要】：背景和目的 隨著光熱選擇性理論的提出,各種新型美容激光機(jī)相繼出現(xiàn),使皮膚血管性疾病的治療更安全有效。本研究擬通過(guò)觀察美國(guó)科醫(yī)人公司生產(chǎn)的維納斯四合一激光機(jī)可調(diào)脈寬倍頻Nd:YAG激光治療皮膚血管性疾病(包括毛細(xì)血管擴(kuò)張癥、血管角皮瘤、會(huì)陰部血管瘤),評(píng)價(jià)其療效及安全性。 方法 采用可調(diào)脈寬倍頻Nd:YAG激光治療毛細(xì)血管擴(kuò)張癥125例、血管角皮瘤24例、會(huì)陰部血管瘤2例,治療參數(shù):波長(zhǎng)532nm,能量12-24J/cm2,光斑直徑3-4mm,脈寬10-20ms。每個(gè)患者每次治療前均拍照存檔,本研究通過(guò)對(duì)比患者治療前后的相片對(duì)療效等級(jí)進(jìn)行評(píng)估,并電話(huà)隨訪其有無(wú)副作用出現(xiàn)及復(fù)發(fā)情況。并用卡方檢驗(yàn)毛細(xì)血管擴(kuò)張癥患者不同治療次數(shù)有效率及治愈率的差異是否有顯著性、顏面部及酒渣鼻性毛細(xì)血管擴(kuò)張癥治愈率的差異是否有顯著性。 結(jié)果 125例毛細(xì)血管擴(kuò)張癥患者中,應(yīng)用可調(diào)脈寬倍頻Nd:YAG激光治療1-5次后,痊愈95例,顯效30例,治愈率76.00%,有效率達(dá)100.00%。比較毛細(xì)血管擴(kuò)張癥患者不同治療次數(shù)有效率及治愈率的差異是否有顯著性顯示:1次與2次及2次與5次治療的有效率、1次與2次及2次與3次治療的治愈率有顯著性差異;2次、3次及4次治療的有效率、3次、4次及5次治愈率無(wú)顯著性差異。顏面部及酒渣鼻性毛細(xì)血管擴(kuò)張癥的治愈率有顯著性差異,前者治愈率較高。僅有2例患者出現(xiàn)暫時(shí)性色素沉著,均無(wú)遺留瘢痕。24例血管角皮瘤患者中,應(yīng)用可調(diào)脈寬倍頻Nd:YAG激光治療1-5次后,治愈24例,治愈率達(dá)100%,均未出現(xiàn)色素沉著、瘢痕或感染等副作用。2例會(huì)陰部血管瘤患兒分別治療3、5次治愈,未出現(xiàn)色素沉著、瘢痕等副作用。電話(huà)隨訪半年均無(wú)復(fù)發(fā)。 結(jié)論 維納斯可調(diào)脈寬倍頻Nd:YAG激光治療毛細(xì)血管擴(kuò)張癥、血管角皮瘤安全有效,值得在臨床中推廣應(yīng)用。維納斯可調(diào)脈寬倍頻Nd:YAG激光可用于治療會(huì)陰部血管瘤。毛細(xì)血管擴(kuò)張癥患者經(jīng)過(guò)2次治療取得明顯效果,經(jīng)過(guò)3次治療大多治愈。隨著治療次數(shù)增加有效率及治愈率有所提高,但統(tǒng)計(jì)學(xué)處理無(wú)顯著性差異。顏面部毛細(xì)血管擴(kuò)張癥比酒渣鼻性毛細(xì)血管擴(kuò)張癥的治愈率高。
[Abstract]:Background and purposeWith the development of the theory of photothermal selectivity, a variety of new beauty laser machines have emerged, which make the treatment of skin vascular diseases safer and more effective.The purpose of this study was to observe the treatment of cutaneous vascular diseases (including capillary angiectasia, angioceratoma) by observing the adjustable pulse width doubling Nd:YAG laser produced by American Medical Company.To evaluate the efficacy and safety of perineal hemangioma.Method125 cases of capillary angiectasis, 24 cases of hemokeratoma and 2 cases of perineal hemangioma were treated with adjustable pulse width doubling Nd:YAG laser. The treatment parameters were: wavelength 532 nm, energy 12-24 J / cm 2, spot diameter 3 ~ 4 mm, pulse width 10 ~ 20 Ms.Each patient was photographed and archived before and after treatment. In this study, the curative effect was evaluated by comparing the images of the patients before and after treatment, and the occurrence and recurrence of side effects were followed up by telephone.Chi-square was used to test whether there were significant differences in the effective rate and cure rate of different treatment times in patients with capillary dilatation and whether there was significant difference in the cure rate of facial part and rosacea capillary vasectasis.ResultAmong the 125 cases of capillary dilatation, 95 cases were cured and 30 cases had remarkable effect after 1-5 times treatment with adjustable pulse width doubling Nd:YAG laser. The cure rate was 76.00 and the effective rate was 100.00000.To compare the effective rate and cure rate of different treatment times and cure rate in capillary dilatation patients showed that the cure rate of 1 and 2 times and 2 times and 5 times of treatment of 1 and 2 times and 2 times and 5 times of treatment with 1 and 2 times and 2 times and 3 times of treatment showed that there was a significant difference in the cure rate of 1 and 2 times and 2 times and 3 times of treatment.There was no significant difference in the effective rate of 3 times, 4 times and 5 times in the treatment of 2 times and 4 times, and there was no significant difference in the cure rate of 3 times, 4 times and 5 times of treatment.The cure rate of facial part and rosacea capillary dilatation was significantly different, the former had higher cure rate.There were only 2 cases of temporary pigmentation, none of them had any scar. 24 cases of hemangikeratoma were treated with adjustable pulse width doubling frequency Nd:YAG laser for 1-5 times, 24 cases were cured, the cure rate was 100%, and none of them had pigmentation.2 cases of hemangioma of pudendal region were treated with scar or infection for 5 times, without pigmentation, scar and other side effects.There was no recurrence in telephone follow-up for half a year.ConclusionVenus adjustable pulse width doubling Nd:YAG laser is safe and effective in the treatment of capillary angiectasis, which is worth popularizing in clinic.Venus adjustable pulse width doubling Nd:YAG laser can be used in the treatment of perineal hemangioma.Capillary dilatation patients after 2 times of treatment achieved obvious results, after 3 times of treatment mostly cured.The effective rate and cure rate increased with the increase of treatment times, but there was no significant difference in statistical treatment.The cure rate of facial capillary dilatation is higher than that of rosacea capillary dilatation.
