【摘要】：報(bào)告1例泛發(fā)性光澤苔蘚并發(fā)毛發(fā)扁平苔蘚�；純号�,4歲。頭皮、頸項(xiàng)部、背部及四肢出現(xiàn)膚色丘疹4年余。皮膚科檢查:頭皮可見毛囊性針頭大丘疹,毛發(fā)稀疏卷曲;頸項(xiàng)、背部及四肢可見針頭大膚色堅(jiān)實(shí)丘疹,有光澤。皮損組織病理檢查:角化過度伴角化不全,基底細(xì)胞液化變性,真皮乳頭淺層半球形浸潤(rùn)性細(xì)胞團(tuán)塊,團(tuán)塊兩側(cè)表皮突延長(zhǎng),呈典型的"抱球狀"外觀;同時(shí)可見毛囊上皮基底細(xì)胞液化變性,周圍可見淋巴細(xì)胞為主的帶狀浸潤(rùn)。免疫組化顯示浸潤(rùn)細(xì)胞以CD4~+細(xì)胞為主。診斷:泛發(fā)性光澤苔蘚并發(fā)毛發(fā)扁平苔蘚。治療:給予他克莫司軟膏,維A酸乳膏外用,轉(zhuǎn)移因子口服液治療,病情好轉(zhuǎn)。
[Abstract]:A case of generalized lichen lustre complicated with hair lichen planus was reported. The child was 4 years old. Skin papules appeared on the scalp, neck, back and extremities for more than 4 years. On the back and on the extremities, the needle had a solid skin colour and a luster. Histopathological examination of the lesions: keratosis with hypokeratosis, liquefaction of basal cells, superficial hemispherical infiltrating cell mass of dermal papilla, and extension of the epidermis on both sides of the mass. Typical "clasping" appearance and liquefaction degeneration of basal cells of hair follicle epithelium can be seen at the same time. Immunohistochemical staining showed that the infiltrating cells were mainly CD4cells. Diagnosis: generalized lichen lustre complicated with hair lichen planus. Treatment: tacrolimus ointment, retinoic acid cream for external use, Transfer factor oral liquid treatment, the condition improved.
