銀屑病患者腸粘膜通透性及病情與飲食相關性的研究

【摘要】: 第一部分進行期銀屑病患者腸粘膜通透性變化的研究 目的:研究銀屑病患者腸粘膜通透性的變化,探討銀屑病的發(fā)病機制。 方法:進行期尋常型銀屑病患者40例,口服測試液50ml(含乳果糖10g、甘露醇5g),留取6h內全部尿液,用高效液相色譜法測定尿中乳果糖(L)與甘露醇(M)濃度,計算二者的比值(L/M),同時測定患者皮損基本消退后的L/M值。以20例健康成人作為對照。 結果:銀屑病患者組L/M比值為0.070±0.026,顯著高于正常對照組(0.029±0.006)及同組患者治愈后(0.036±0.009)(P0.05)。 結論:進行期尋常型銀屑病患者腸粘膜通透性較正常人及皮損消退后患者增高,可能與銀屑病的發(fā)生及發(fā)展有關。 第二部分銀屑病患者病情與飲食相關性的研究 目的:探討影響銀屑病病情的飲食因素及其他相關因素。 方法:采用問卷調查表,應用1:l配比病例對照研究,共調查200例尋常型銀屑病患者的飲食及其他病情相關因素。使用單因素logistic回歸分析結合多因素條件logist ic回歸模型分析。 結果:面粉(OR=2.214)、牛肉(OR=3.308)、羊肉(OR=2.145)、魚蝦(OR=1.952)、辣椒(OR=2.559)、洋蔥(OR=1.519)、吸煙(OR=2.515)、飲酒(OR=2.205)、肥胖(OR=1.557)、咸菜酸菜(OR=1.752)是銀屑病發(fā)病的可能危險因素。經(jīng)常食用大米(OR=0.422)、粗糧(OR=0.252)、蛋類(OR=0.459)、鮮奶(OR=0.576)、蘋果(OR=0.650)、香蕉(OR=0.656)、芹菜(OR=0.515)、茄子(OR=0.652)可能是銀屑病發(fā)病的保護因素。 結論:吸煙、嗜酒、魚蝦、羊肉、牛肉、辣椒、面粉、肥胖、洋蔥、咸菜酸菜可作為獨立危險因素,對銀屑病的發(fā)病有一定的影響。大米、粗糧、雞蛋、鮮奶、蘋果、香蕉、芹菜、茄子可能對銀屑病的發(fā)病起保護作用。
[Abstract]:Part one study on the changes of intestinal mucosal permeability in patients with psoriasis. Objective: to study the changes of intestinal mucosal permeability in patients with psoriasis and explore the pathogenesis of psoriasis. Methods: in 40 patients with psoriasis vulgaris, the concentrations of lactulose 10 g and mannitol 5 g / L were determined by high performance liquid chromatography (HPLC). The ratio of the two was calculated and the L / M value of the patients after the skin lesions had basically subsided was measured. 20 healthy adults were used as the control. Univariate logistic regression analysis combined with multivariate conditional logist regression model was used.
